A: REQUIREMENTS FOR DIPLOMATE COURSE:

1. Resume with 2x2 colored picture.

(Include present employment and clinic schedules)

2. Attachments of the following:

- Certificate of PCOM Membership

- Certificate of Completion of the PCOM Basic Course or the BOSH

- Certificate of Good Standing from PMA and PCOM

- Certificate of Employment duly signed by HR or Management as

proof of active OH practice

B. REQUIREMENTS FOR CONFERMENT TO DIPLOMATE STATUS:

1. Must be in Good Standing with PMA and PCOM

2. Must have passed the Written and Oral Examinations

3. Must be in at least 2 years of active OH practice

4. Must have completed 150 units in 3 years through the following:

- Annual Convention = 30 CME Units

- Midyear Convention = 20 CME Units

- RTD’s = 5 CME Units
