AFFIDAVIT OF DISCREPANCY

I, FRANCISCO GABARDA BATALLER JR., 37 years old, married,

and presently residing at #366 Brgy. Talaga, Tanauan City, Batangas,
after having been duly sworn to in accordance with law do hereby depose
and state THAT:

1. Sometime in February 2018, I proceeded to the Land

Transportation Office (LTO), San Pablo City, Laguna to process the
renewal of my Professional Driver’s License;

2. After submission and compliance with the necessary

documents for the renewal of my driver’s license, the said license was
printed with the surname “BATALIER”;

3. I only noticed that my surname was misspelled when I went

out of the LTO Office;

4. Upon seeing that my surname was misspelled I immediately

returned my license to the personnel in charge of the printing to correct
the same, however, I was advised to execute an affidavit to correct my
surname as printed;

5. Attached herewith is a copy of Voter’s ID as ANNEX “A” to

prove and show that the correct spelling of my surname is BATALLER
and not BATALIER;

6. I am executing this affidavit to attest to the truth of the

foregoing fact that my surname’s correct spelling is BATALLER and not
BATALIER, and for the purpose of correcting my surname as printed in
my professional driver’s license, and other legal intents and purposes
this affidavit may serve.

IN WITNESS WHEREOF, I hereunto affixed my signature this 8th

day of March 2019 at Tanauan City, Batangas.
