Home Visitation Report

When we visited Mrs. Chona Belarma, a mother of Job Belarma, a Grade 7 Rose , just to check and had a follow-
up on his son because of his Absenteeism” and his low academic performances in school during the first semester in the
school year. Accordingly, his son was daignosed and due to his health condition, the reason of his absenteeism and low
grades.

When we had our follow-up and visited Lorence Soberano, Grade 7 Rose as we clarify the reason of
absenteeism and low performance in class. From him, he is not any ore interested in going to school instead he just
decide to be a bystander and his parents and her sister knew it. But we were able to talk to him and said he will go back
in school and cope up the missing quizzes and classess. And we agreed, that he will attend Remedial Classess.

Home Visitation Report

When we visited Norhaimen Tabina, of Grade 7 Rose, during the period of having low grades, low scores in quizzes due
to his absenteeism. We agreed together with his parents that he will go back in school and attend remedial Classes for
him to cope up the missing scores, and can pass all subjects.

During our Home visitation of our student, Mariana Grace Autor, due
to her absenteeism that resulted to low grades and low scores. We
agreed together with his parent that he will join and attend Remedial
Classes and answer the remedial activities.
