    Greetings in peace!

    It is of my great respect to inform you that I am currently on sick leave for


    the past three months, due to my thyroid problem. I have recently gone through
    series of tests and the doctor has advised me to take more rest and medication
    for complete recovery. I am attaching my current medical report for your
    reference.

    Thank you for your always giving me your understanding with regards to my
    situation and I would like to request your good office if I could extend once again
    my leave from April 01, 2019 to May 31, 2019.
