AFFIDAVIT OF EXPLANATION

I, JOHN DOE, Filipino, of legal age, and presently residing at Zone 1

Grotto, Brgy. ACDFAe, City of FAFSCA, AHSX, after having been duly sworn to in
accordance with the law, do hereby depose on oath and say that:
1. I am the brother of deceased XXX who died on August 13, 2018.

2. Based on my knowledge, considering the fact that I had lived with him
since 1985, he only had two (2) children whose names and dates of birth
are as follows:

3. I affirm that XXX had only been married to YYY and that no other
marriage was entered into by him.

4. I have no knowledge of any person under the name of XXX who is

claiming to be related to my deceased brother.

5. I am executing this affidavit to attest to the truth of the foregoing facts

and for whatever legal purpose it may serve.
