ACKNOWLEDGEMENT

Preparing a research paper is truly a challenging task for anyone. Being students
of Science, Technology, Engineering, Agriculture, and Mathematics (STEAM) we
accepted it with great challenge and opportunity. We humbly present this research paper
that we have suitably prepared and submitted to meet the partial fulfillment of our
INQVEST subject.

We owe a deep gratitude to Asst. Prof. Jin H. Genove, our INQVEST adviser for
her patience in guiding us and her intellectual supervision in every aspect of our paper.
This work would not have been duly made without her valuable directions for us to be
able to finish our work.

We extend our sincere gratitude to Mr. Roberto Montebon of Silliman University

Medical Center and Mrs. Felisa B. Pa-alan of Silliman University Medical Center from the
research committee, the head nurse of the third floor nursing ward, for their suggestions

kindly participated in our survey questionnaires.
