Overview

The Patient Record System is integrated with all the associated medical information of every
student in Cavite State University Cavite City Campus.
The main functionalities available in this system are:
 Maintaining the basic information and student number of each student.
 Maintaining the medical history of every student.
 It is a password protected system and only authorized users can access the records.

All these features include the ability to create, update, retrieve information through search results
and to edit obsolete records.

Objective
Our purpose of creating the Patient Record System is to lessen the workload of our medical
consultant/nurse in Cavite State University CCC. Also to offer a good data management system
that enables faster and more efficient storage, retrieval and easy updating of patient records.
Graphical User Interface

Figure 1. This is the Login form of the system, here we can put the Username And Password of
the authorized user to be able to access the database.

Figure 2. If we want to see the inputted password, we can tick the Show Password checkbox to
be able to see the inputted password.
Figure 3. If the correct username and password has been inputted, When pressing the Login
button this message box will appear.

Figure 4. After successfully login, this loading form will pop out.
Figure 5. This is the main form of the system, here we can see the different buttons such as the
New Patient, Update Patient, Delete Patient, Medical Record list, Refresh, Log-Out, and Search. We can
also see here the current date and time for the day.

Figure 6. In this figure, it shows that when the Clear button has been clicked, it will clear all the
information inputted in all the textboxes in the Patient Register Form.
Figure 7. Here a message box will appear if the Save button have been click with a missing
field(s) in the Patient Register Form.

Figure 8. When the New Patient button in the main form is clicked, the Patient Register form
will appear. Here we can put the basic information of every student. By pressing the Save button the
message box in the figure will appear.
Figure 9. After Pressing the Save button in the Patient Register, The Medical Record Form will
Show. Here the authorized user can input the medical information of the student based on his findings.

Figure 10. Just like in the Patient Register Form, by inputting the medical information of the
student and pressing the Save button the information will also be saved and a message box will appear
indicating that the record was added.
Figure 11. In this figure, it shows that the information recorded in the System can be searched
through the Search button.

Figure 12. By pressing the Update Patient button in the main form, this Form will show. In here
the authorized user can Update the records if it does become obsolete.
Figure 13. While for the Patient Delete button, the authorized user can easily delete
unnecessary info or mistaken records .

Figure 14. In this figure it shows the Medical Record List form, it appears when the Medical Record List
Button in the main form has been clicked. All of the students Medical History can be found here and it
also includes a search button for easy retrieval of medical info.
Figure 15. When pressing the Log-Out button in the main form, this Message box will appear.

It will prompt to Ask yes or no if ever the authorized user wants to log-out from the system.

Figure 16. This is the Database of the System, through the use of MS Access the system have efficient
storage and retrieval of information.
Figure 17. The authorized user can also update the information through the database just by selecting
the certain rows and columns in the table he wanted to update.
