Figure 1 shows the landing page of Rescar Dental Care Clinic where public user may view
or browse all the features of the system.

Figure 2 shows the sign-up form of Rescar Dental Care Clinic where public user must
sign-up to have an account in order for them to log in and make an appointment.

Figure 3 shows the log in page of Rescar Dental Care Clinic where user who already have
an account can log in to make an appointment.

Figure 4 shows the dashboard of the patient. In the content of the patient dashboard, you
can there the word Please complete your profile. Which mean you must first complete
your information before making an appointment.

Figure 5 shows the information of the patient. You can see there the Update Profile button
which is if you click that you will see the other information that is required for you to
make an appointment.
After filling up the required information you can make appointment already.

Figure 6 shows the appointment of the Rescar Dental Care Clinic where user will choose a
day that she/he want to schedule his/her appointment where the Dentist or Doctor is
available that day. If the doctor is available just click the button BOOK NOW.

Figure 7 shows the appointment details of the Rescar Dental Care Clinic where user will
input the symptom and the reason of the symptom before clicking the word make
appointment. After that you can now see your appointment list.

Figure 8 shows the appointment list of the patient where you can see there the name of the
patient, schedule day, schedule date, start time and the end time. You can also see there
the word print. If you want to print your appointment details just click the square button in
the bottom of the word print.

Figure 9 shows the appointment details of the patient where if you want to print just click
the button print this page.

Figure 10 shows the Log Out button of the patient. If the user wants to log out their
account, they will just click the log out button.

Figure 11 shows the log in page of the dentist or doctor where doctor must log in to go to
the doctor dashboard.

Figure 12 shows the dashboard of the doctor where you can see there the Patient List and
the doctor schedule.

Figure 1shows the Log Out button of the doctor. If the doctor wants to log out her account,
she will just click the log out button.

Landing page
For you to view the landing page you must click the link
http://clinic/landingpage.php the user will direct to the landing page of Rescar Dental Care
Clinic.
Features of Landing Page
 About – This link/tab consist the important details or information about Rescar
Dental Care Clinic
 Account – This feature consists the Login and Sign up.
Login – If you already have an account, you may log in by inputting your
IC number and password and you will directly go to the dashboard of the
patient.
Sign up – If you don’t have an account, you can create an account by
filling up the required information that attach in the form.
Appointment button – By clicking the appointment button you will see the appointment
form where you need to choose a day, date and time that you want the you will see there if
the availability of the doctor is available. If it is available just click the BOOK NOW
button.
Book now button – After clicking the book now button you will see there the
appointment details where you must input the symptom and the reason. After that click the
button make appointment. After that you will see your appointment list.
Appointment list – In the appointment list if you want to print your appointment details,
just click the square button in the bottom of the word print then after that you will your
appointment details. If you want to print out just click the button print this page.
Log out – To go to the log out just click Benhur Piedad in the right side to see the log out
button.
Admin link – To see the admin link, scroll up the landing page then you will see the
admin link in the bottom right side. After that click the admin link then you will directly
go to the log in page of the doctor.
Log in for doctor – To log in doctor must input his/her doctor id and password then click
the button log in. After logging in you will see the doctor dashboard.
Doctor Dashboard – In the doctor dashboard you can see in the side menu bar the Doctor
schedule and the Patient List.
Doctor Schedule – If the doctor will set a schedule she/he must click the Doctor schedule
then input the schedule that his/her availability is available.
Patient List – If the doctor wants to see the patient who make an appointment, he/she
must click the Patient List.

LOG OUT – If the doctor wants to log out she must click the name Sheila Marie Rescar
then you will see there the log out button then click it.
