CHECKLIST OF BID DOCUMENTS FOR PROCUREMENT OF GOODS

Envelope 1 –

1. Philgeps Certificate of Registration (Platinum Category)

2. Statement of on-going, completed and awarded gov’t & private contracts within
the period stated in the BDS including contracts awarded but not yet started, if any;
and statement of single largest completed contract similar to the contract to be bid

3. The prospective bidder's computation for its Net Financial Contracting Capacity
(NFCC) or a commitment from a Universal or Commercial Bank to extend a credit
line in favor of the prospective bidder if awarded the contract to bid (CLC).
4. Notarized Bid Securing declaration or any of the following:
Bid Security as to form, amount and validity period;
a) Cash, Manager’s Check, Cashier’s Check, Irrevocable Letter of Credit or
Bank Draft – Two Percent (2%) of ABC

b) Bank Guarantee – Two Percent (2%) of ABC

c) Surety Bond – Five Percent (5%) of ABC

5. Conformity with technical specifications, as enumerated and specified in Sections VI


and VII of the Bidding Documents; and
6 Notarized Sworn Statement by the prospective bidder or its duly authorized representative in
the form prescribed by the GPPB as to the following:
( a ) The signatory is the sole proprietor or duly authorized representative of the
prospective bidder;
( b ) The signatory is granted full power and authority to do, execute and perform
any all acts necessary and/or to represent the prospective bidder in the bidding,
with the duly notarized Secretary’s Certificate attesting to such fact, if the
prospective bidder is a corporation, partnership or joint venture;
( c ) It is not “blacklisted” or barred from bidding by the GOP or any of its agencies,
offices, corporations, or LGUs, including foreign government/foreign or
international financing institution whose blacklisting rules have been recognized
by the GPPB;
( d ) Each of the documents submitted in satisfaction of the bidding requirements is
an authentic copy of the original, complete, and all statements and information
provided therein are true and correct;
( e ) It is authorizing the Head of the Procuring Entity or his duly authorized
representative/s to verify all the documents submitted;
( f ) It complies with existing labor laws and standards.
( g ) It complies with the responsibilities of a prospective or eligible bidder provided in
the PBDs; and
( h ) It complies with the disclosure provision under Section 47 of the Act in relation
to other provisions of R.A. 3019;

Envelope 2 –

1. The financial proposal submission sheet, which includes the bid prices and the bill of
quantities for procurement of goods, and scope of work for procurement of services,
and the applicable price schedules
