Part I – Legal documents
Registration certificate from SEC, Department of Trade and Industry (DTI) for sole
1 proprietorship, or CDA for cooperatives
Mayor’s/Business permit issued by the city or municipality where the principal place of
2 business of the prospective bidder is located, or the equivalent document for Exclusive
Economic Zones or Areas
Part II – Technical Documents
Statement of the prospective bidder of all its ongoing government and private contracts,
3 including contracts awarded but not yet started, if any, whether similar or not similar in nature
and complexity to the contract to be bid
Statement of the bidder’s Single Largest Completed Contract (SLCC) similar to the
4 contract to be bid
A valid Philippine Contractors Accreditation Board (PCAB) License or Special PCAB License
5 in case of Joint Ventures, and registration for the type and cost of the contract to be bid
Part III – Financial Documents
Audited financial statements, showing, among others, the prospective bidder’s total and
current assets and liabilities, stamped “received by the BIR or its duly accredited and
6 authorized institutions, for the preceding calendar year which should not be earlier than
two(2) years from the date of bid submission
7 Computation for its Net Financial Contracting Capacity (NFCC)
8 Financial Bid Form, which includes bid prices
Other Documents:
Tax clearance per E.O. 398, 2005, as finally reviewed and approved by the Bureau of Internal
9 Revenue (BIR)
10 Latest income and business tax returns
11 Certificate of PhilGEPS Registration
12 Bid Security in the prescribed form, amount and validity period
13 Omnibus Sworn Statement in accordance with Sec. 25.3 of IRR
 It is not blacklisted or barred from bidding
 Each of the documents submitted is an authentic copy of the original, complete, and
all statements and information provided therein are true and correct
 It is authorizing the Head of the Procuring Entity or his duly authorized
representative/s to verify all the documents submitted.
 The signatory is the duly authorized representative of the prospective bidder, and
granted full power and authority to do, execute and perform any and all acts
necessary and/or to represent the prospective bidder in the bidding
 It complies with the responsibilities of a prospective or eligible bidder provided in the
PBDs
 It complies with existing labor laws and standards
 It is not related to the Head of the Procuring Entity (HOPE), members of the BAC,
TWG and BAC Secretariat
14 Organizational chart for the contract to be bid
15 List of contractor’s personnel with complete qualification and experience data
List of contractor’s major equipment units, which are owned, leased, and/or under purchase
16 agreements, supported by proof of ownership or certification of availability of equipment from
the equipment lessor/vendor for the duration of the project
