Your child has been chosen to be a part of the Collegiate Prep Choir for
the 2017-2018 school year!
Students had to pass an audition, exhibit excellent classroom behavior, be able to follow
directions, and be able to work as a team. We will perform at certain events throughout the
year and you will be given prior notification. There will be a $20.00 cost for this group for
new members, which covers the shirt for choir, awards, and parties.
Practices are held Wednesday afternoons from 3:15-4:15 in the music room. The first
practice will be next Wednesday, October 4, 2017.
Attendance is mandatory should your child choose to participate and punctuality is very
important. Although I know unexpected events may happen and will of course be excused,
please note that three unexcused absences or tardies will result in your child being removed
from choir.

Attached are the permission form, contract, and t-shirt order form, which must be returned
to me by Wednesday, October 4, 2017. I am looking forward to an exciting year at school
and in choir!

Collegiate Prep Choir Contract 2017-2018
This form is due back by October 4th.
All choir students are expected to have excellent behavior. We represent the entire Collegiate Prep Elementary
School when we perform and we need to set a good example for the rest of the student body. During a rehearsal,
students are expected to stand and sing without talking to one another. Long periods of talking AND singing can
cause strain to young vocal cords. It also slows down the rehearsal and makes it “boring” for everyone! One warning
note will be issued for inappropriate behavior so that parents can be made aware of any issues when they occur.
Please take a few moments to review with your child the expectations we have for them as a choir member as stated
below. Students who cannot follow directions in the rehearsal or the classroom will be removed from the choir. I am
looking forward to a great year!
Please sign after reading, stating that you and your child have read and understand the commitment to choir.
I agree to keep talking, playing, or socializing to a minimum during practice. Repeated interruption will result in a
written warning note home. If the behavior occurs again, I will be removed from choir.
I agree to work as a team and to be willing to sit, stand, or work with any member of choir even if I don’t know them
very well.
I realize that I must be in the carline on time to pick up my child. If I am late more than three times, I will be
dismissed from choir. Parents are required to be in the car rider line at 4:15 on their Choir rehearsal day. Do not enter the
building to pick up your child. We will walk the students to the car rider line area at 4:15 and loading will begin.
Gum, cellphones, and food are not allowed in practices. Cellphones are allowed outside in the carline if your child
needs to contact you.
I will work hard to memorize the song lyrics, and be willing to study them at home.
I agree to wearing my choir uniform for every performance. All Choir participants must purchase a t-shirt and will be
required to wear their choir shirt with khaki pants at all performances.
My child and I have reviewed and agree to the rules and requirements for Collegiate Prep Choir. She/He understands
that choir is a 3rd,4th,and 5th grade honor choir, and it is a privilege to represent the school and participate in this
program.
Everyone in choir must order a t shirt, which is included in the $20.00 fee.
Please fill out this form and turn in with your money no later than Wednesday,
October 11th. Thank you!
