Doctors to be assigned at the clinic are Occupational Medicine and Consultant Specialist.

1.2.1 Three times (3x) a week clinic schedule for four (4) hours per visit, with a total of twelve
(12) hours per week for Occupational Medicine Specialist and Consultant;

In case the doctor shall be required to go on duty during legal or special holiday, special
rates shall be applied for.

1.2.2 Medical consultation and check-up coverages:

a. Issuance and granting of medical certificate
b. Evaluating medical certificate submitted by employees prior to approval
c. Evaluation and approval of SSS, medical and related forms for sick leave claims
d. Hospital or home visit as maybe deemed necessary shall be billed separately based
on the regular doctor rate unless it falls on a holiday, special rate shall be applied for.

1.2.3 Emergency treatment of work connected injuries

1.2.4 Preparation of patient and his medical history in case of referral to an
Outside Medical specialist
1.2.5 Evaluation of pre-employment and annual physical examination

1.2.6 Pre-marital/family planning counseling and certification
1.2.7 Conduct lectures for medical update purpose
