MEMORANDUM OF IN CRAFTING THE SCHOOL IMPROVEMENT PLAN

This Memorandum of Agreement is entered into this 7th of April 2017 in Lubao National High School by
and between:

The LUBAO NATIONAL HIGH SCHOOL (LNHS), with School Identification Number 300894, a public high
school, with principal address at San Nicolas 1st Lubao, Pampanga, represented in this Agreement by its
LNHS Principal, RODELIO T. LAXAMANA, Filipino, of legal age, and hereinafter referred to as the SCHOOL;

-and-

School Head Teachers and Subject Leaders with members who are recognized and designated officers of
the school located at San Nicolas 1st Lubao, Pampanga conducted on April 17. Represented in this
Agreement by Sir/Madam ANGELITA P. TURLA, hereinafter referred to as the SCHOOL HEAD.

WITNESSETH:

WHEREAS, the LUBAO NATIONAL HIGH SCHOOL, hereinafter referred to as “SCHOOL”, is the primary
government instrumentality mandated to formulate, implement, and coordinate policies, plans, programs
and projects for the establishment and conduct of Strategic Planning and crafting of School Improvement
Plan – Annual Improvement Plan (SIP-AIP) for the school year 2017- 2018;

WHEREAS, the SCHOOL strategic planning can be attended and joined by the local levels to take into
consideration the needs and views of the community and the stakeholders;

WHEREAS, the STAKEHOLDERS will participate in the full implementation of the Strategic Plan and SIP-AIP
of the school year 2017- 2018;
