Clear.: XXXml/min Shows the last measured clearance value. Displayed for
1 minute after a successful measurement.
Plasma-Na: XXXmmol/l Shows the plasma sodium concentration of the last single
measurement for 1 minute.
Kt/V: X.XX Shows the current Kt/V. Displayed for 1 minute when the UF
goal has been reached or if the OD senses clear fluid.
Kt/V: Not available There are no Kt/V data available as the calculation of Kt/V was
aborted. Displayed for 1 minute when the UF goal has been
achieved or if the OD senses clear fluid.
HDF/HF < > OCM An OCM and an HDF or HF treatment procedure cannot be
performed simultaneously. Displayed once if OCM is selected
while HDF/HF is active. If HDF/HF is selected while OCM is al-
ready active, the OCM will be aborted and the message “OCM
off” will be displayed. The OCM can, however, be reselected
after HDF/HF has been switched off.
SN < > OCM The OCM cannot be started as a Single Needle treatment (SN
or SN click-clack) has been selected.
Profile< > OCM An OCM cannot be started in conjunction with a Na-/UF profile
1, 5 or 6 and a treatment time of less than 180 minutes. Dis-
played when the OCM is switched on and/or when selecting a
profile.
OCM off/X The OCM was aborted due to a malfunction and will be deacti-
vated until the end of the treatment.
Inform the service technician of the error code (X).
OCM adjust cancelled The OCM adjustment was not successfully completed and will
be repeated after the next treatment.
OCM adjust intended Displays when the optical detector senses dark fluid and if an
adjustment measurement is required on completion of the
treatment in progress.
Input value too low The operator has attempted to enter a value for an OCM para-
meter that is below the minimum limit.
Input value too high The operator has attempted to enter a value for an OCM para-
meter that is above the maximum limit.
1.1.2 Warnings
Kt/V too low Displays if the prescribed Kt/V cannot be reached at the end of
the treatment with the current Kt/V.
OCM OFF The OCM option will be deactivated until the machine is
switched off.
– Power failure with battery backup
– CPU II error
OCM OFF/2 The OCM option will be deactivated until the machine is
switched off.
One of the CD cells has measured a value below 11.5 mS/cm
during more than 3 consecutive OCM measurements.
– Check CD cells for proper connection
– P.C.B. LP 633-5 defective
OCM OFF/3 The OCM option will be deactivated until the machine is
switched off.
– System error
OCM OFF/5 The OCM option will be deactivated until the machine is
switched off.
A downward pulse occurs although an upward pulse was re-
quested.
– Check the bibag/concentrate delivery
– Check the concentrate pumps / mixing system
– Air in the dialysate
OCM OFF/6 The OCM option will be deactivated until the machine is
switched off.
– System error
OCM OFF/7 The OCM option will be deactivated until the machine is
switched off.
Implausibility of the ratio of blood flow to clearance or no appa-
rent change on the CD cell during more than three consecutive
measurements.
– Poor terminal connection of the CD cell
OCM tech. deact./1 The OCM option is decalibrated and can only be reactivated
after a successful pulse calibration.
Deviation between the 8-bit and the 12-bit CD value of the pre-
cell > 0.5 ms/cm for > 20 s.
– Disinfection
– Check OCM calibration and calibrate, if necessary
OCM tech. deact./4 The OCM option is decalibrated and can only be reactivated
after a successful pulse calibration.
Three cyclic pressure holding tests (CPHT) showed a deviation
of > 0.5 ms/cm between CD cell 7 and CD cell 110 for > 4.5 s.
– Disinfection
– Check OCM calibration and calibrate, if necessary
OCM tech. deact./7 The OCM option is decalibrated and can only be reactivated
after a successful pulse calibration.
More than 5 consecutive measurements showed a clearance of
more than 1.3 times the blood flow or below zero.
– Check the CD cells for poor terminal connection.
– Disinfect, if necessary
– Check the calibration
OCM tech. deact./8 The OCM option is decalibrated and can only be reactivated
after a successful pulse calibration.
Zero pulse measurement failure or time of maximum pulse be-
tween pre- and post-CD cell > 13 s.
– Check the hydraulic for proper flow
– Check OCM calibration and calibrate, if necessary
OCM tech. deact./9 The OCM option is decalibrated and can only be reactivated
after a successful pulse calibration.
The correction factor of the zero pulse measurement for upward
or downward pulses is outside the acceptable range of 0.8 to
1.2.
– Perform a disinfection
– Check OCM calibration and calibrate, if necessary

Caution
The accuracy of the measuring equipment used during the calibration procedure
is decisive for the accuracy of the OCM measurement.
The measuring equipment used for the calibration of the conductivity must have
an accuracy of 0.05 mS/cm within a temperature range of 35 °C – 39 °C.
We recommend to use the UMED universal measuring device which is available
from Fresenius (part no. M32 403 1).

In 4008 H/S machines it is generally possible to define a correction factor for
all clearance measurements. This feature has been provided to be able to
adapt a machine at its place of installation to latest knowledge gained from
scientific discussion that has not been completed yet. Based on present know-
ledge the correction factor for Fresenius Polysulfone membranes must be ad-
justed to “1” (factory setting). The accuracies specified are only applicable for
Fresenius Polysulfone membranes in combination with a correction factor that
was set to “1”.
