AFFIDAVIT OF SOURCE OF INCOME

I, HENRY CABELIN DELANTAR, of legal age, Filipino, married, and

a resident of Cluster 2 Purok 6, Sagkahan, Tacloban City, after having been
duly sworn to in accordance with law, hereby depose and state:

1. That I am earning money as an on-call welder and “latero” to different

employers or clients;

2. That my monthly income ranges from P 15,000 pesos to P 20,000

pesos on pakyaw basis depending on the work or project given to me
by an employer or client;

3. That I am making this affidavit to attest to the truth of the foregoing

facts and for whatever legal purpose this may serve.
