having been sworn in accordance with the law, hereby depose and state That:

1. I am the duly authorized representative of Supreme Power Corporation, the owner

of a Fuel Transfer Pump and Unloading Pump House to be erected located at Brgy.
Santo Rosario, Capas, Tarlac;

2. The estimated amount of building/structure/facility to be erected is PESOS: Two

Seventy-Six Thousand Nine Hundred Sixty-Four Pesos and 17/100 (Php
276,964.17); and

3. I declare, under the penalties of perjury, the above information is verified by me,
and to the best of my knowledge are true and correct or based on authentic
documents.
