COMPLAINT-AFFIDAVIT

I NASHARELLA TORRES, of legal age, Filipino citizen, and resident of Barangay

Nayubyuban, Naguilian, La Union, hereby aver the following facts:

1. I am the owner of Torres General Merchandise;

2. The Civil Service Commission- Regional Office 1 posted in PhilGEPS regarding the
procurement of their office supplies in the amount of ONE MILLION FIVE HUNDRED
THOUSAND PESOS (Php 1, 500, 000.00);

3. I submitted my intention to participate in the bidding process. I submitted the

necessary documentary requirements four (4) days before the scheduled bidding;

4. The scheduled public bidding for the supplies was set on October 24, 2019 at 10:00
AM;

5. On October 25, 2019, one Ivan Padilla, an employee of the Civil Service Commission
Regional Office 1 and the officemate of Dennis Dwane Macanas, called and told me
that there is an important matter that he needs to discuss with me;

6. Later of the said day, we met at Max’s Restaurant in San Fernando City, La Union.
He told me that he saw Mr. Macanas opened my bid and disclosed the same to Ms.
Alyssa Joy Marzo, another bidder for the same contract, in order to lower the bid. He
even showed to me the picture showing the act of Ms. Macanas opening my bid,
which is prior to the scheduled opening of bids. Herein attached is the picture
marked as Annex “A”;

7. Mr. Dennis Dwane Macanas is the Administrative Officer III of the Procurement Staff
under the Management Services Division, and is also a member of the Bids and
Awards Committee of the CSC-Regional Office 1;

8. I am filing against Mr. Macanas for Grave Misconduct and Conduct Prejudicial to the
Best Interest of the Service. It is without prejudice to the filing of criminal action as
the facts mentioned may warrant.

9. I, the undersigned, executed this affidavit to attest to the truthfulness of the foregoing
facts and to support the filing of an administrative complaint, and/or criminal
complaint, as the facts may warrant.
