I, TERESITA C. AUJERO, of legal age, Filipino, married and a resident of Purok

Pag-Asa Visayan Village, Tagum City, Davao del Norte, after having been duly sworn to
in accordance with law, do hereby depose and say:

That I am the mother of MA. CRISTINA CAYSIDO AUJERO;

That my daughter is a planholder of Philam Plans;

That on September 13, 2007 she passed away as evidenced by the Death
Certificate duly issued by the National Statistics Office (NSO), currently known as
Philippines Statistics Authority (PSA);

That based on the records of the Certificate of Death, her fullname is recorded as
“MA. CRISTINA CAYSIDO AUJERO”;

That based on her record in Philam Plans, her fullname was recorded as “MARIA
CRISTINA C. AUJERO”;

That “MA. CRISTINA CAYSIDO AUJERO”and “MARIA CRISTINA C. AUJERO”

refers to one and the same person;

That I am executing this affidavit to attest to the truth of the foregoing facts, to
inform the proper authorities, especially the Philam Plans, relative to this matter
and for whatever legal purpose this may serve best.
