profiles of senior officers or its equivalent rank and personnel involved in the
appraisal work
11 Resume/Personal Data Sheet of the officers and staff involved in the
appraisal work
12 Photocopy of Certificate of Training/Seminar attended issued by the
organization sponsoring the seminar/training
13 Sworn Corporate Secretary’s Certificate, duly notarized, confirming the
following:
a. Absence of any pending criminal or civil actions against or any bankruptcy
or insolvency proceedings involving any directors, executive officers or the
company;
b. Sworn undertaking by the Board of Directors or any authorized officer that
the company would not appraise assets where they have conflict of
interest
14 Statement of all its ongoing and completed government and private contracts
for the last three (3) years including contracts awarded but not yet started, if
any, whether similar or not similar in nature and complexity to the contract to
be accredited.
15 Certification from clients, preferably banking institutions within the last five (5)
years stating that they have availed of the services of the appraisal company
and that the service rendered was satisfactory, together with an authority for
Pag-IBIG Fund to inquire if the appraisal company had any financial
transaction with them.
16 List of clients within the last five (5) years who have availed of the services of
the appraisal company, including information on the client’s contact person
and numbers.
17 Certificate of good standing from any DTI-recognized appraisal associations
namely, Institute of Philippine Real Estate Appraisers (IPREA), and Philippine
Association of Real Estate Appraisers, Inc. (PARA), where they are members.
18 Audited Financial Statements for the last two (2) years stamped received by
Bureau of Internal Revenue (BIR) or its accredited and authorized institutions
19 Proof of membership of all coverable employees and qualified workers with
Pag-IBIG Fund including payment of contributions and employer’s counterpart
