VI.

DISCHARGE PLAN

Diet

 Instruct patient to eat/drink:

o Healthy foods that are accessible and locally available in the market like
vegetables and fruits such as malunggay, pechay, alugbati, water spinach
(kangkong), cabbage and dahon kamote, orange, banana, soursop (babana)
and papaya
o 6 - 8 glasses of water everyday
o Encourage to cook dishes with a small portion of meat

 Instruct client to avoid:

o Eating of dried fish (uga) everyday

 Sample Meal Plan

 Breakfast: 1 Cup of Rice

Egg Omelette

1 Glass of Milk

 Lunch: 1 Cup of Rice

Bas-uy

1 Banana

1 Glass of Fruit Juice

 Dinner: 1 Cup of Rice

Fish Tinola

1 Glass of Water

Hygiene

 Instruct the patient to do the following:

o Proper Skin Care


 Cleanse body with appropriate body soap
o Routine hair, eyes, nose, mouth and foot care:
 Comb hair regularly
 Wash hair everyday with shampoo
 Wash face using washcloth with water or mild soap
 Brush teeth every after meal
 Trim the nails straight across
 Keep the nails clean and healthy

o Instruct patient to check the genital area for signs of redness, soreness,
irritation and infection
o Instruct patient not to use harsh soap or alcohol wipes in cleaning the genital
area
o Instruct patient to apply powder on areas with creases such as neck, elbow,
knee, and etc.
Activity

 Instruct patient to avoid the following:

o Strenuous activities (running, squatting, jumping)


o Lifting heavy objects
o Cigarette smoking and electronic smoking (vaping)
o Drinking alcoholic beverages (beer, wine, gin)
 Teach client to sleep for 8 – 10 hours at night and take naps for 1 – 2 hours in
the afternoon
 Inform client not to have sexual intercourse until 6 weeks postpartum

Medications

 Instruct client to take medications as prescribed by the physician

o Ferrous Sulfate 1 tablet 2x a day by mouth for 1 month


o Cefuroxime 500 mg 1 tablet 2x a day by mouth for 1 week
o Multivitamins + Iron 1 tablet 1x a day by mouth for 1 month
o Paracetamol 500 mg 1 tablet by mouth PRN
Community Support

 Instruct patient/significant others as to do the following:

o Access the primary health care provider in barangay health centers

o Avail of the health services such as medicines, prenatal check-ups,


immunizations and etc. offered by the nearest health centers

o Record all the contact numbers of family members, neighbors, midwives and
health agencies in case of emergency

o Keep medical records and forms in a safe and secure place

Spiritual Support

 Advice the live – in partner and his family to take quality time to the patient

 Encourage patient to express her feelings and concerns to prevent thinking of


negative thoughts

 Encourage client to do meditation and personal reflection to prevent stress

 Encourage live – in partner and his family to exhibit positive environment to the
patient

 Instruct live – in partner and his family to attend to her needs and be there if she
wants to vent out during the recovery period

 Teach patient on certain diversion activities

Financial Resources/Needs

 Suggest the patient to avail of PhilHealth insurance and membership benefits


 Instruct the patient to:

o Collaborate with SWA regarding the settling of bills


o Ask assistance to the health workers for the settlement of hospital billing,
medications and home care needs
o Ask financial assistance from their willing family members for the bills,
needed medication of the patient, other home and health care needs.
o Assist patient to the option where she could save best without compromising
her health
Appointment

 Follow up check-up after 6 weeks postpartum


 Consult a health care provider in the health center regarding the process of
effective conception
 Instruct patient to complete future prenatal check – ups:

o Once a month every 1st trimester (1st to 3rd months)


o Twice every 2nd trimester (4th to 6th months)
o Every week starting the 8th month of pregnancy
