DISCHARGE PLAN
 Continue home medications.
MEDICATION
Instruct the client to take the following medications as prescribed:
(Tumaren agitoy nga agas):
 Ascorbic acid plus zinc, 1 tablet twice a day (agtumar ti maysa a tableta
maminduwa ti maysa aldaw; 8am ken 8pm)
 Tramadol 50 mg/tab, 1 tablet thrice a day as needed for pain
 Cefuroxime 500mg/tab, 1 tablet thrice a day for 7 days (agtumar ti
maysa a tableta ti mamintallo ti maysa nga aladaw agarop pito ng
aladaw; 8am-12pm-8pm)
 Mupirocin ointment apply on operative site twice a day
 Metformin 500 mg/tab, 1 tablet once a day for 1 month (agtumar ti
maysa a tableta maminsan ti maysa a aldaw ti makabulan; 8 am)
 Feboxustat 40 mg/tab, 1 tab once a day at PM for 1 month (agtumar ti
maysa a tableta maminsan ti maysa a aldaw ti makabulan; 8 pm)
 Fenofibrate 160/tab, 1 tab once a day at PM for 1 month (agtumar ti
maysa a tableta maminsan ti maysa a aldaw ti makabulan; 8 pm)
 Atorvastatin 20 mg tab once a day at bed time for 3 months (agtumar
ti maysa a tableta ti maysa nga aldaw sakbay maturog agarop tallo a
bulan; 9 pm)
 Amlodipine 5 mg/tab 1 tab once a day PM for two weeks (agtumar ti
maysa a tableta iti rabii agarop duwa dominggo; 8 pm)
 Losartan 50 mg/tab 1 tab once a day, AM (agtumar ti maysa a tableta
iti bigat agarop duwa dominggo; 8 am)
Physical Exercises (with caregiver/significant other):
EXERCISE  Walking every early morning for at least 30 minutes.
 Seated exercises: marching, turning the upper body from side to side,
raising the heels and toes, raising the arms towards the ceiling, raising
the opposite arm and leg, bending the legs, clapping under the legs,
bicycling the legs, making circles with the arms, practicing moving from
sitting to standing.

Brain Exercises (with caregiver/significant other):

 Learn something new, such as a second language or a musical


instrument.
 Play board games with your kids or grandkids. Or get your friends
together for a weekly game of cards.
 Work on crossword, number, or other kinds of puzzles.
 Read, write, or sign up for local adult education classes.

 Advise patient and significant others to take home medications


TREATMENT
regularly as prescribed. Right time, right dosage and right route
must be strictly followed. (Tumaren/ipatumar iti usto nga oras

 Comply with treatment regimen.
 Emphasize proper diet; LSLF. (Haan unay ti natataba ken naa-
apgad a makan.)
 CBG monitoring and record. (Kanayon nga icheck ti blood sugar
ken i-record)
 Routinely do a wound dressing on the operative site. (Dalusan
ken agasan kanayon ken nasayaat iti sugat ti pasyente)

Patient is advised to:


HEALTH TEACHING
• Always be with the patient. Do not leave the patient. (Kanayon
kadwaen ti pasyente. Haan nga panpanawan iti pasyente.)
• Take medications as prescribed strictly. (tumaren agiti agas nga
inbaga ti doctor)
• Assist patient when moving.
• Eat his meals three times a day. (mangan ti tallo nga beses ti
maysa aldaw)
• Take his medications as prescribed strictly. (tumaren agiti agas
nga inbaga ti doctor)
• Have an adequate rest. (paginanaen ti pasyente iti nasayaat)
• Ask assistance from significant others when moving.
• Practice slow deep breathing to promote relaxation and prevent
stress. (Aganges-anges ti nauneg ken iwasan ti stress)
• Encourage to take a bath daily. (digosen ti pasyente iti kanayon)
• Encourage routine and reminders to the client and relatives to

facilitate adherence.

• Teach about the right time to take his medications as well as other

measures that will be advised.


• Instruct relatives not to stress the client.

• Providing a calm environment and significance others should be


aware and instructed for his to take enough rest periods.
The patient should attend his follow-up checkup in order for the

physician to see the progress of the patient and to advise what to do.

 Follow-up check-up at the follow up after 1 week at Gabriela

Hospital (November 19, 2021 at 8AM) to DR. VIADO.

 Follow-up after 1 month at Gabriela Hospital (December 12,

2021 at 2PM) to DR. BUTARDO.

 Continue diet as tolerated. Eat fruits and vegetables.


DIET
 Continue Low Salt Low Fat (LSLF) Diet. (madi ti naaap-gad
ken natataba)
 Never skip a meal. (haan nga aglagtaw nga mangan)
 Instructed to increase oral fluid intake. (kaadwen ti uminom ti

danom)

 Avoid taking sweet foods. (haan unay mangan ti nasasam-it)

 Don’t smoke and drink alcohol. (madi ti agsigarilyo ken

uminom)
 The patient should be in a safe environment upon discharge.
SAFETY AND SECURITY
SPIRITUAL  Wear comfortable clothes and avoid tight clothing. (agbado ti

komportable nga bado)

 Keep environment clean and away from dangerous materials.

(iyadayo dagiti banag nga maka-disgrasya)

 Client’s relatives should be one call away if he needs assistance.

(kinanayon dapat nga adda kadwa iti pasyente)

 Always communicate to a Higher Being. Ask for strength,

guidance and good health as he continues the battle of his life.
