DISCHARGED PLAN
MEDICATION • Insulin 70/30 15 units 30 minutes
before breakfast; 18 units 30 minutes
before dinner
• Metformin 500 mg + Vildagliptin 50
mg tab BID
• Atorvastatin 40 mg tab OD HS
• Carvedilol 6.25 mg/tab BID
• Clopidogrel 75 + ASA 75 mg tab OD
in AM
• Trimetazidine 35 mg/tab 1 tab BID
• Captopril 25 mg/tab ½ tab BID with
BP precaution
• Pantoprazole 40 mg/tab 1 tab OD in
AM
EXERCISE • Exercise at least 30 minutes daily.
• Be sure to monitor blood sugar levels
before and after all exercise routines
to better understand how your body
responds to exercise.
TREATMENT • Instruct the patient/ significant others
of the patient to take her medications
on time.
• Be cautious to the things that may
affect the recovery.
• Insulin therapy
• CBG monitoring pre-breakfast and
pre-dinner.
• Daily wound dressing on diabetic
foot.
HEALTH TEACHING • Take medications on time.
• Administration of proper injection
site, if the significant others doesn’t
know yet how to inject an insulin to
the patient you can teach them how to
do it accurately.
• Note for the results of your CBG
monitoring, so that you will visit your
physician you can show to her the
results of it.
• Remind the patient to have adequate
rest and sleep.
• You should have with you your
proper wound supplies for your daily
wound dressing.
• Wash your hands thoroughly before
cleaning the area of wound.
• Use a sterile gloves.
• Use saline water on the tape of the
existing wound dressing, then inspect
it.
• Gently clean the wound using gauze
and saline water.
• Place the new dressing.
• Be gentle when bathing your feet.
• Keep your feet warm and dry.
• Avoid promptly the stressors and
stimuli that may affect you.
• Contact your physician immediately
if there’s any unnecessary symptoms
and signs.
OUT PATIENT • Follow up check up at OPD on
January 16, 2021 at 10 am.
• Request for 2D Echo Doppler
Sonography
• Follow up Dr. Anastacio at St. James
Hospital once with result of 2D Echo
Doppler Sonography.
• Follow up Dr. Realin at OPD once
with result of Duplex Ultrasound on
left leg.
DIET • Eat vegetables, fruits, nuts, seeds,
legumes, potatoes, whole grains,
breads, herbs, spices, fish, seafood
and extra virgin olive oil.
• Eat chicken, eggs, cheese, yogurt
moderately.
• Eat red meat once in a while.
• Do not eat sugar-sweetened
beverages, added sugars, processed
meat. Refined grains, refined oils and
other processed foods.
SAFETY AND SECURITY SPIRITUAL • If you feel something unnecessary,
contact your physician immediately
or tell it to your significant others.
• Do not forget to pray.
