Earthquake Assessment Report

Date of Inspection : October 30, 2019

Ma’am:

Reference is made in our visual site team inspection dated October 30, 2019 together with the City
Engineering Inspectorate Team, BFP and CDDRMC. During our team visual site inspection, we noticed
that the building is safe to use. Please refer below our inspection report.

INSPECTION & OBSERVATION REPORT:

1. Minor cracks on masonry walls (hairline cracks).


2. Minor damage/misalignment of door jamb (one door only).
3. Minor damage of tiles.
4. Minor cracks of concrete slab on grade. (Slight movement of slab)
5. Minor cracks of floor topping on second floor slab.

RECOMMENDATION & TO COMBAT THE ISSUE:

In general, after experiencing a strong earthquake happened last October 29, 2019 with
magnitude 6.6 and the succeeding aftershocks, the integrity of the structure of the said building as
stated above was in good condition, the minor defects can be repaired through rehabilitation in
accordance to engineering standards and specifications. Therefore, the said building can be used or safe.
