Hypothesis

Alternative Hypothesis-
-There is significant difference between the effect of Rasnasaptak Kwath and Vishwadi
kwath in Katigraha.

4 Introduction Vata, Pitta and Kapha are the three doshas mentioned in Ayurveda. These
tree doshas either vitiated or decreased causes the vydadhi. Vata, the one among the
three doshas is said to be superior most in all aspects as it affects the action and
movement of the other two doshas as well as all the dhatus etc. Vata is mainly present in
Pakvashay,KATI, sakthi,shrotra,asthi,sparshanendriyam.

Katigraha is considered as one among the 80 Vata Nanatmaja vikara by


Sharagdhara Samhita. Katigraha is explained as separate disease in Ayurvedic classical
text ‘Gada Nigraha’. The lakshana of Katigraha are Shoola and Sthambha/graha in Kati
Pradesh. When Vata vitiated by many hetus enters into Kati Pradesh, it produces the
symptoms like shoola and sthabdhata/graha in kati Pradesh, that condition is called as
Katigraha.
Locomotion is one of the prime necessities of every human being. Katigraha is
one of the vatavyadhi which affect the normal function of locomotion. Even though this
disease, not being a life threatening one, it hampers daily activity of the person. According
to a survey Katigraha i.e. low backache is extremely common and second only to the
common cold. It is very common in both male and female equally.

NEED FOR THE STUDY


The number of patients suffering with Katigraha is increasing day by day. It
occurs due to stressful work hours, improper seating posture during work, sitting in one
posture for a long time, lack of stretching/exercise, long travels on 2/4 wheelers etc. It can
be considered as low backache, which disturbs the routine work of an individual.
As the medical science recognized the severity of katigraha, medicine which
relieves the pain, stiffness, improves the functional ability, restores functional disability and
controls the condition with cost effectiveness is the need of the century.
Ranasaptak Kwath and vishwadi kwath are mentioned in Sharangdhar Samhita
and sahastrayoga respectively for treatment of all kinds of vata vyadhi including katigraha.
To find out the more effective medicine among the two which will help in reduction of
symptoms of katigraha fastly, with no complications is necessary so that it can be more
precisely used in the treatment of katigraha,making it the drug of choice in katigraha
among these two, this study is necessary.

5 Review of The literature for my dissertation will be reviwed from Ayurvedic classical textbooks,

5.2 Disease review- Katigraha is explained as separate disease in Ayurvedic classical
text ‘Gada Nigraha’. Katigrahais one of the 80 Nanatmaja Vatavyadhi mentioned by
Acharya Sharangadhara. None of Brihatrayee has explained Katigraha as a separate
entity. But a brief description of Katigraha as a disease with its Lakshana and Chikitsa is
available in Bhavaprakasha, Yogaratnakara and Vrindamadhava has given about the
same symptoms of Katigraha and describes its treatment in detail 5.
In Charaka Samhitha, Sushrutha Samhitha and Astanga Hrudaya, in Nanatamaja
Vata Vyadhi though Katigraha has not been mentioned, other symptoms of spinal diseases
like Prusthagraha, Trikagraha, Manyagraha etc are found, and also mentioned as a
symptom in some diseases.

Shuddha saam vata when entered in the Katipradesha causes pain,stiffness


then it is called as Katigraha.
1. The conceptual part of literary review of Katigraha from Ayurvedic aspect will be
elaborated in detail.
2. Previous information of Katigraha will be collected from research articles, journals
and internet.

Kwath of both will be prepared by the method of preparation of Kwath


described in Sharangdhar Samhita dravya will be taken 1 part, 16 parts of water will be
added and boiled till 1/8th part remains.
All the drugs will be taken in equal quantity for both the kwathas. 20 grams of drugs will be
taken and 320ml of water will be added and boiled till 40 ml kwath remains. The remaining
kwath will be filtered and will be asked to drink in tolerable form.
The procedure of making kwath will be given to the patients which is mentioned in
adjoining annexure.

6.2 Secondary Objectives-

1. To study Katigraha from Ayurvedic classical texts


2. To study the karmukatva of Rasnasaptak Kwath and vishwadi kwath in detail from
classical texts.
3. To study the effect of Rasnasaptak kwath and Vishwadi Kwath on signs and symptoms
of Katigraha.

7 Methodolog 7.1 Type Of Study Design- Randomized open comparative clinical study.

7.2 Location Of Study-
For this study subjects will be selected from O.P.D/I.P.D of our college

7.3 DURATION OF STUDY-

Duration of administration of drug- 60 days, follow up on 15th, 30th, 45th & 60th day.
Total duration of study and observation- 18 months

7.4 Method of selection of study subjects-


a)Inclusion Criteria-
1. Age group between 20-70
2. Both genders i.e. male & female
3. Presence of classical features of katigraha
4. Patients of katigraha fulfillimg the samprapti of katigraha i.e shuddha aam vata
enterimg in the katipradesh causing pain,stiffness

b)Exclusion Criteria-
1. Patients With known case of low backache due to any
infection,cancerous,congenital,vascular diseases.
2. Patients associated with any known case of other systemic disorder.
3. Pregnant or lactating women.
4. Patients not willing for trial.

7.5 Method of selection of comparison-

The sample size for present study will be 60. Further those patients will be matched and
divided into two equal groups viz. group A & group B.

8.2 Sampling Technique-

Simple random sampling technique(SRS)- 60 Patients will be randomly selected from


OPD/IPD of our college

8.3 Methods For Data Collection-

Data will be collected with the help of history taking,physical examination of the patients. It
will be done at day 0 and at every follow up and will be noted in follow up table.

Collected data will be presented in the form of graphs, tables and charts.

8.6 Plans for Statistical Analysis-

Wilcoxon signed rank test will be used for efficacy of drugs at 5% level of
significance.
For comparative study of drugs Mann Whitney ‘U’ test will be applied at 5%
level of significance. Other suitable test will be applied whenever necessary.

8.7 Overall Assessment Criteria-

Marked improvement 75% to 100% relief in the symptoms


Moderate improvement 50%-74% relief in the symptoms
Mild improvement 25 %to 49% relief in the symptoms
Poor improvement <25% relief in the symptoms

Withdrawal Criteria-

1. It may be considered as – or + 5 another patients


2. Patients who will discontinue the treatment
3. Patients unwilling to take the treatment during study
4. Patients showing any type of adverse effect

8.8 Observation & Results-

Results and inference will be drawn on the basis of readings obtained before and after
treatment by using statistical methods.

8.9 Conclusion-

Final conclusion will be drawn on the basis of statistical analysis with overall assessment.
