We , the researchers of Grade 12 STEM of Sta. Catalina National High School.Conducting
research study entitled of “Impact of Using Technology in Education at SCNHS S.Y. 2019-2020 “
a quantitative research as one of the requirement in the subject of Practical Research II .
In this regard ,we would like to as your permission to allow us to conduct a survey among the
teachers and students in SCNHS . The survey would last only about 10-15 minutes and would be
arrange at a time convenient to the teachers and students schedule (during break time).
Participation in the survey is entirely voluntary and there are no known anticipated risk to
participation in this study . All information provided will be kept in utmost confidentiality and
would be only use for academic purpose .
