AFFIDAVIT OF EXPLANATION

I, MITCHOTTE APOSTOL TAMSI, of legal age, married, Filipino citizen and a

resident of Liquicia, Caba, La Union, state under oath that:

That I worked as BUS SUPERVISOR for Abu Obaida Bin Al Jarah Private
School, Ajman, United Arab Emirates since 2017 until July 2019;

That during my tenure in Ajman, U.A.E., with the said company, I meet an
accident which causes my right knee injury;

That on July 7, 2019, I arrived in the Philippines and undergo further tests and
examination for my right knee injury. I was advised to undergo MRI for my right knee
by my attending physician;

That I was not able to earn during my employment in U.A.E. and my

employment was cancelled because of my right knee injury;

Further, I was not able to go back to U.A.E. and/or seek any job for now due
to my right knee injury

That I am executing this affidavit in accordance with law and based on my

personal knowledge for other legal purposes it may serve.
