Pinagkawitan, Lipa City, do hereby depose and state that:
1. I am the Attorney-in-Fact of Marigrace Mauhay as shown in
“Annex A”;
2. My principal is an owner of a house and lot Sorrento
Subdivision located in Brgy. Pinagkawitan, Lipa City
3. As part of the process, a title will be issued in the name of
the owner;
4. In view of the foregoing, I manifest that my principal agree
that the title will bear the name Marigrace Palicpic Mauhay as
the owner;
5. I execute this affidavit freely and voluntarily and for whatever
legal purpose this may serve.
