 ENTRANCE EXAM PROCEDURE
1. Secure a copy of Carlos Hilado Memorial State College Entrance Test Application Form (CHMSCETAF) from the Office of the
Guidance Services or Download CHMSCETAF from the official Facebook page https://www.facebook.com/chmscofficialpage/.
2. Fill out ALL required entries in the CHMSCETAF. (LRN Strictly Required)
3. Submit the filled-out CHMSCETAF at the office of the Guidance Services with the following requirements: (a) 1 pc Long Brown
envelope; (b) 2 pcs Recent 2x2 ID Picture.
4. Secure your CHMSCET schedule from the Guidance Personnel.

Instruction to Examinees During Exam

1. Examinees should report one (1) hour before the exam schedule. LATE EXAMINEES WILL NOT BE ADMITTED.
2. Bring the following:
a. CHMSETAF Student’s Copy
b. Pencil with eraser
c. School I.D.
3. CELLULAR PHONES AND OTHER ELECTRONIC GADGETS ARE STRICTLY PROHIBITED. Wear the prescribed dress code.

5. Take the CHMSCET. (You are only allowed to take CHMSCET once in a year. Retaking of test will invalidate your test score.)
6. Get result on intended date of release. Results must be claimed by the examinee personally, otherwise a duly signed authorization letter must
be presented.
7. Proceed to the College of your choice for further screening procedures.
