AFFIDAVIT OF LOSS

We, RENATO MARTINEZ and WILFREDO MARTINEZ, both of legal

age, Filipino, married, with residence and postal address at Brgy. Mudboron,
Alangalang, Leyte and Brgy. Cogon, Palo, Leyte, respectively, after having been
duly sworn in accordance with law, do hereby depose and say:

1. That we are the owners of a certain parcel of land, commonly known

as Lot No. 352-D-3 (Lot No. 352-D-2-B) covered under TCT No. T-28830,
containing an area of more or less 70 square meters;

2. That we are representatives of the Heirs of Teofila Martinez, under

whose name the above-described property is registered;

3. That on the basis of the Decision Based on Compromise

Agreement issued by the Regional Trial Court Branch 43 of Tacloban City on
November 24, 2017, the above-described property was adjudicated in our favor;

3. That the home of our cousins Virginia Martinez and Ester Martinez,
where the original OCT was kept, burned down last April 23, 2013 along with all
their possessions to include the original OCT; hence, we have considered the same
to be lost beyond recovery;

4. That we are executing this affidavit to attest to the truthfulness of

the foregoing facts and for whatever legal purpose this may serve;
