SUBJECT: Explanation Letter for Flag Raising Ceremony for the month of
September 2019

DATE: 28 October 2019

1. Reference:

a. Attendance Sheet for Flag Raising Ceremony dated September

9, 23 and 30, 2019.

2. In connection with the above reference, the undersigned forgot to affix his
signature but attended the said flag raising as per DTR on 23 September 2019, while on
the other hand, he was on leave on 30 September 2019.

However, the undersigned, in his honest recollection, barely missed the 09

September 2019 flag raising event due to sudden technical glitch occurred at the LRT rail
system that alighted passengers unexpectedly. The undersigned had to take a different
mode of transportation causing time delay.
