Pursuant to the Memorandum from the CHED Officer-in-Charge dated 5 March 2o18
Reiterating and Enforcing the Guidelines and Procedures with Respect to Foreign Trips
of Government Officials and Personnel Under the Executive Department, In
Accordance with the Issuances on Foreign Travels by the Office of the President, this
Commission and Other National Government Agencies," State Universities and
Colleges are hereby informed of the requirements for securing Authority to Travel
Abroad, as follows:

i. Travel Authority
2. Evaluation Form for the Necessity of Foreign Travel for State Universities and
Colleges
3. Assessment of International Conference/Meeting issued by International
Affairs Staff-Cl-lED
4. Board Resolution

Failure to comply with these requirements shall be sufficient grounds for the denial or
non-processing of said requests. The process flow for securing the request for foreign
travel is attached as Annex A. The Evaluation Form for the Necessity of Foreign Travel
is attached as Annex B.

For personal travel, SUC personnel shall submit to the SUC Personnel Office the
approved Leave of Absence and a Certification stating that the travel is personal in
nature and that the personnel concerned shall not in any way represent the institution
on Official Capacity during the approved Leave of Absence. OOO153ft
