AFFIDAVIT OF CONSENT
FOR TRAVEL OF A MINOR

I, SALVACION AVILA ADVINCULA, of legal age, widow, Filipino citizen and

a resident of Brgy. Pancian, Laoag City, Philippines, under oath, hereby depose and state
that:

I am the legal guardian of CHRISTINE MAE A. GARAÑA, a minor being born

on October 30, 2006, being the biological grandmother of the aforesaid minor exercising
substitute parental authority as her biological mother, my daughter, is already dead and
her biological parents were not legally married;

CHRISTINE MAE A. GARAÑA, is going for a trip abroad specifically to Hawaii,

U.S.A. for her to permanently stay therein with her paternal biological grandfather;

For this purpose, I have sufficient and adequate financial capacity to support and
defray the said travel and do hereby undertake to finance her trip abroad from the
application fees, airline tickets, board and lodging, pocket money and such other
expenses that she may incur so that she would neither be a burden to the State nor at the
place of destination at any time during her trip abroad;

Further, my minor granddaughter, CHRISTINE MAE A. GARAÑA will be

accompanied and attended to by her biological father CHRISTOPHER A. GARAÑA;

I am giving my full consent to CHRISTOPHER A. GARAÑA to accompany and

attend to my minor granddaughter CHRISTINE MAE A. GARAÑA in her travel abroad;

I am executing this Affidavit to declare the truth of the foregoing facts and for
whatever legal purpose it may serve.
