AFFIDAVIT OF CONSENT
FOR TRAVEL OF A MINOR

I, MICHELLE NICOLAS BAUTISTA, of legal age, Filipino citizen and a resident of Brgy.
54-A, Lagui-Sail, Laoag City, Philippines, under oath, hereby depose and state that:

I am the biological mother of MYEISHA LIAN NICOLAS BAUTISTA, a minor being

born on 31 January 2008, a citizen of the Republic of the Philippines, with Passport No.
P2120306A;

MYEISHA LIAN N. BAUTISTA, is going for a trip abroad specifically to Abu

Dhabi,U.A.E. this June, 2018 to August, 2018, for leisure/vacation purposes;

For this purpose, I have sufficient and adequate financial capacity to support and
defray the said travel and do hereby undertake to finance her trip abroad from the
application fees, airline tickets, board and lodging, pocket money and such other expenses
that she may incur so that she would neither be a burden to the State nor at the place of
destination at any time during her trip abroad;

Further, my minor daughter, MYEISHA LIAN N. BAUTISTA will be accompanied and

attended to by her aunt MARICEL NICOLAS PINAROC, of legal age, and with Passport No.
P6709400A, in her travel abroad;

I am giving my full consent to my daughter MARICEL NICOLAS PINAROC to

accompany and attend to my minor daughter MYEISHA LIAN N. BAUTISTA in her travel
abroad;

I am executing this Affidavit to declare the truth of the foregoing facts and for
whatever legal purpose it may serve.
