Leave of Absence

checklist
*Do not use for Illness Protection Plan (IPP) or Short Term Disability (STD) absences.
Please obtain the applicable Associate Care Guide.

We get it: life happens! Some meaningful events or unexpected


disruptions mean you’ll have to take a step back from work for a
while. We’ve created this handy checklist to help you submit a
request for a Leave of Absence from work so that you’re free to
focus on what matters most.

Absences longer than five (5) days may qualify for a Leave of
Absence. Leaves of absence may include, but are not limited to:
• Maternity Leave
• Parental Leave
• Compassionate Care Leave
• Family Medical Leave
• Reservist leave
• Other legislated leaves
• Personal unpaid Leaves
• Medical Leave – Full Time hourly Associates may be
eligible to apply for Illness Protection Plan (IPP) or
salaried associates for Short Term Disability (STD)

NOTE: Requesting a leave of absence does not guarantee that it will be approved. Associates are required to provide supporting
documentation when requested by Walmart.

Requesting an LOA
□ Review Make sure you have your Walmart Identification Number (WIN) handy.
□ Call the absence reporting line at 1-833-Wal-Well (1-833-925-9355) OR sign on to www.walmartlivewell.ca
and follow the prompts.
□ Enter your WIN and birthdate, so the system knows it’s you.
□ Follow along with the prompts to request your leave of absence.
□ Your manager will be notified of your request.
□ Contact your manager to discuss your request.
□ Provide supporting documentation if requested by your Manager.

IMPORTANT: Supporting documentation is required for all Medical Leave of Absence requests.

Reviewing your LOA


All leave of absence requests are reviewed by management. The status of your request can be confirmed by:
□ You will Signing on to www.walmartlivewell.ca
□ Reviewing your leave by selecting View Absences/Leave Requests.
□ The website will show whether your Leave of Absence is approved, declined, or pending.
□ If you are unable to access the website, confirm the approval of your leave directly with your manager.
If your LOA is Approved/Acknowledged
□ Throughout your leave, you may need to continue to provide supporting documentation when requested.
□ Return to work when expected or request a modification or extension to your leave.

If your LOA is Declined


□ Remain at work or return to work immediately.
□ If you disagree with the decision, speak with your manager directly.

NOTE: If your Leave of Absence is in a “Pending” status and you submitted the request more than 2 days prior, follow up
with your manager directly to confirm any outstanding information required to complete their assessment of your request.

If you want to cancel, extend or shorten your LOA


□ Throughout Once your leave is approved/acknowledged, only your Manager can cancel your request on your behalf.
□ If you need to extend or shorten your leave, this can be done online or by phone by following the prompts AND contacting
your manager directly to determine if updated documentation is required to support the modification to your request.

Getting ready to return to work


□ Remain Contact your manager at least 2-3 weeks prior to your anticipated return to work date (when possible)
to confirm your return to work date.
□ Advise your manager immediately if you will need to extend your leave or if you will require accommodation
upon your return to work.

Your responsibilities
 Always return calls or emails from Walmart within 24 hours.
 Advise Walmart immediately when there is any change to your leave of absence.
 Ensure that Walmart has your current address, telephone number(s) and email address during any leave of absence
so you can be reached if needed. You must immediately provide updated contact information to Walmart in the event
that such information changes.
 Continue to pay benefit premiums to maintain your benefit coverage while on leave. A letter will be sent to your home
with instructions on how to set up your payment for your premiums, if applicable.

IMPORTANT: Benefit coverage will continue in accordance with Walmart’s Leave of Absence Policy, and for any period
approved (provided you continue to pay any applicable benefit premiums within the specified timelines). It is your responsibility
to request an extension of your leave of absence if unable to return to work by your anticipated return to work date to ensure
the continuation of benefit coverage.

NOTE: You may be eligible to apply for Employment Insurance (EI) benefits from Service Canada for some legislated leaves.
