In accordance with the provisions contained in Rule 9 of the Lok Sabha Secretariat
(Recruitment and Conditions of Service) Rules, 1955 read with R&CS Order
No. PDA·1104/2004 dated the 17th December, 200-4,the Competent Authority has decided
that the orders regarding empanelment of Hospitals and Diagnostic Centers under CGHS~
Delhi for treatment and diagnostic procedures and fixation of package rates, as contained in
the Government of India, Ministry of Health & Family Welfare
D.M F. No. Rec.1-2004/JD(Gr)/CGHS/Delhi/CGHS(P)(2} dated the 31st October, 2008,
1st December, -2008, 10th December, 2008 & 12th December,2008, as reproduced in
Annexure-l to Annexure-IV respectively to this Order read in conjunction with the General

purpose of providing Central Government Health Scheme (CGHS) coverage to family
members of the CGHS covered employees subsequent to implementation of
recommendations of the Sixth Central Pay Commission, as contained in the
Governmentof India, Ministry of Health & Family Welfare O.M No. S.11012/1/98-CGHS (P)
dated the 10th December, 2008, as reproduced in Annexure to this Order read in conjunction
with the General Adaptation Order No.185 dated the 19th December, 1957 shall apply to the

Subject: Revision of income limit for dependency for the purpose of providing
Central Government Health Scheme (CGHS) coverage to family
members of the CGHS covered employees subsequent to
implementation of recommendations of the sixth Central Pay
Commission -jregarding
3. " On the basis of the recommendations of the s'"Cl'C, the Department of Pension
and Pensioners' Welfare under para 5.5 of thdr*OM No. 38/37/OS-P&PW (A) dated
2.9.2008, has fixed the amount of minimum pension as Rs. 3500/~ per month and under
para 8.10f this OM the amount of family pension has been fixed as 30% of the basic pay
in all cases shall be subject to a minimum of Rs.3500/- per month and maximum of 30%
of the highest pay in the Government. The DoP&PW at para 8.5 of the same OM has
decided that dependency criteria for the purpose of family pension shall be the minimum
family pension along with dearness relief thereon.
5. As such, all the orders related to the CGHS Rules stands amended to the extent
that the income limit for Rs.1500/- per month from all sources including pension! and
family pension stands amended to an income of Rs. 3500/- glus amount of the dearness
relief on the basic pension of Rs.3500/- as on the date of consideration. The amount of
dearness relief, as indicated in the income limit stands for the amount of dearness relief
drawn by a pensioner! family pensioner on the date of consideration and not the amount
of dearness relief due on the date of consideration.

6. The income limit for dependency of"Rs. 3500/- eh~ amount of the dearness relief
on the basic pension of Rs.3500/- as on the date of consideration". shall also be
applicable for the cases covered under CS (MA) Rules, 1944 for the purpose of
examining eligibility of family members of the Central Government employee for
medical facilities under the Rules.
8. This issues with the concurrence of the Department of Expenditure vide their LO.
No. 566/EV 12008 dated 4. I J .2008.

SUbject: Relaxation of procedures to be followed in considering requests
for medical reimbursement.
The undersigned is directed to slate that under the extant instructions, a CGHS card
holder, who wishes to apply for reimbursement of the expenditure incurred by him I her on
medical treatment of either self or his I her dependent family members, the present
reimbursement procedure needs verification of bills and issue of essentiality certificate by
the treating doctor, and the Medical Superintendent of the hospital. The process of
verification of bills and issue of essentiality certifICates are time consuming with the doctor at
times being busy or being away from office for whatever reason. This necessitates repeated
vislts to the hospital for getting the verifICation done and essentiality cerufcate obtained.
Representations have been received in the Ministry of Health & Family Welfare requesting
for doing away with the two requirements and for the Ministries I authorities concerned to
verify and check the authenticity of the claims on the basis of the prescription slip and the
diagnostic report submitted by the Government servant I pensioner. In the event of any
doubt. the concerned Ministry I Authority can alWays get verification done from the hospital
concerned.
2. The undersigned is also directed to state that CGHS guidelines currently provide for
relaxation of guidelines to cover full reimbursement in individual cases depending upon
merits of each case. In the case of Hon'bJe Members of Partiament. the powers to relax the
guidelines have been delegated to the Lok Sabha Secretariat and Rajya Sabha Secretariat
respectively and in the case of Hon'ble Chief Justice of Supreme Court and judges of the
Supreme Court to the Secretary Genera' of the Supreme Court.
(1) it has now been decided to do away with the procedure for verification of bills and
issue of essentiality certificate by the treating doctor, and the Medical Superintendent of the
hospital. Ministries I authorities concerned may verify and check the authenticity of the
claims on the basis of the prescription slip and the diagnostic report submitted by the
Government servant f pensioner. In the event of any doubt, the concerned Ministry I
Authority can always get verification done from the hospital concerned. Modified
reimbursement claim form, alongwith checklist is annexed.
(2) All cases involving requests for relaxation of rules for reimbursement of full
expenditure will henceforth be referred to a Technical Standing Committee, to be chaired by
the DGHS I Addl.OGHS and consist of Director (CGHS) and subject matter specialists. If
the Technical Standing Committee recommends the relaxation of rules for permitting full

reimbursement of expenditure incurred by the beneficiary, the full reimbursement may be
allowed by the Secretary (Health & Family Welfare) in consultation with IFD. A check list for
consideration of requests for reimbursements in excess of approved rates may include:
(a) The treatment was. obtained in a private non-empanelled hospital under emergency
and the patient was admitted by others when the beneficiary was unconscious or severely
incapacitated and was hospitalised for a prolonged period;
(b) The treatment was obtained in a private non-empanelled hospital under "emergency
and was admitted for prolonged period for treatment of Head Injury, Coma, Septicemia,
Multk>rgan failure, etc.;
(c) The treatment was obtained in a private non-empanelled hospital under emergency
for treatment of advanced malignancy;
(d) The treatment was taken under emergency in higher type of accommodation as
rooms as per his! her entitlement are not available during that period;
(e) The treatment was taken in higher type of accommodation under specflc conditions
for isolation of patients to avoid contacting infections;
(f) The treatment was obtained in a private non-empanelled hospital under emergency
when there is a strike in Government hospitals;
(9) The treatment was obtained in a private non-empanelled hospital under emergency.

2004 and~1ath October, 2005, issued by the Ministry of Health & Family
Welfare, vide which orders were issued revising the rates of monthly
.subscription for availing CGHS facility, as also the entitlement for free diet,
entitlement of accornmodation in private empanelled hospitals under CGHS,
etc. On the basis of the recommendations of the Sixth Central Pay
Commission, pay and allowances of serving employees and pension of civil
pensioners / family pensioners have been revised by the concerned
authorities.
2. The upward revision of pay I pension of doctors and other staff in the
CGHS has increased the cost of operating the Central Government Health
Scheme. It has, therefore, been decided to revise the rates of
subscriptions, to be made by employees / pensioners, for availing
benefits under the CGHS, with effect from 1$ June, 2009. It has also
been decided to revise the monetary ceiling limits for various entitlements of
the beneficiaries for avaiiing CGHS faciiities.
The monetary ceiling for free diet for CGHS beneficiaries is revised to
pay I pension / family pension of Rs. 7,450/-per month.
(0) Monetary ceiling for free diet for beneficiaries suffering from TB
or mental disease}:
The monetary ceiling for free diet in case of beneficiary suffering from
TB or Mental disease is revised to pay 1 pension I family pension of Rs.
11,1601- per month
4. It is clarified that the reference to pay in this order relates to the pay
drawn in the pay [band.

Pensioners have an option to get their CGHS pensioner card made by
either making CGHS contribution on an annual basis (twelve months) or by
making contribution for 10 (ten) years [120 (one hundred and twenty)
months] for getting a pensioner CGHS card with life-time validity. It is
clarified that:
(ii) Pensioner beneficiaries, who have already obtained CGHS card with
life time validity by paying a lump sum amount equivalent to 10 years'
contribution, will not be required to pay any additional amount as a result of
the revision in the rates of contribution for availing CGHS facility;
(v) Any pensioner I family pensioner who is entitled to avail CGHS facility
has not so far got his I her pensioner CGHS card made, the rate of
contribution in such cases will be with reference to the grade pay that he I
she would have drawn in the post held by him I her (at the time of his I her" .

(d) Hospitals / diagnostic centers empanelled under CS(MA) Rules, 1944 shall
not charge more than the package rates.
(e) Expenses on toiletries, cosmetics, telephone bills etc. are not reimbursable
and are not included in package rates.
In case, there are no CGHS prescribed rates for any test/procedure, then AIIMS
rates are applicable. If there are no AIIMS rates, then reimbursement is to be
arrived at by calculating admissible amount item-wise(e.g. room rent,
investigations, cost of medicines, procedure charges etc) as per approved
rates/actually, in case of investigations.

6. (a) CS(MA) beneficiaries are entitled to facilities of private, semi-private or
general ward depending on their basic pay. The entitlement is as follows:-
(c) The package rates prescribed arc for semi-private ward. If the beneficiary
is entitled for general ward there will be a decrease of 10% in the rates; for private
ward entitlement there will be an increase of 15%. However, the rates shall be
same for investigation irrespective of entitlement, whether the patient is admitted
or not and the test, per-se, does not require admission.
8. (a) The m(t:-;imurn room rent for different categories would be:
(b) Room rent mentioned above at (a) above is applicable only for treatment
procedures for which there is no CGHS prescribed package rate. Room rent will
include charges for occupation of Bed, diet for the patient, charges for water and
electricity supply, linen charges, nursing charges and routine up keeping.

should have furnishings like wardrobe, dressing table, bed-side table, sofa
set, etc. as well as a bed for attendant. The room has to be air-conditioned.
(e) Semi Private ward is defined as a hospital room where two to three
patients are accommodated and which has attached toilet facilities and necessary
furnishings.
(f) General ward is defined as halls that accommodate four to ten patients.
10. The recognised hospitals shall honor permission letter issued by competent
authority and provide treatment/investigation facilities as specified in the
permission letter.
11. In case of emergencies, the beneficiary shall have the option of availing
specific treatment/investigation from any of the above mentioned hospitals of
his/her choice (provided the hospital is recognised for that treatment
procedure/test), on production of valid ID card, issued by competent authority.
12. During the in-patient treatment of the CS(MA) beneficiary, the Hospital will
not ask the beneficiary or this attendant to purchase separately the

medicines/sundries/equipment or accessories from outside and will provide the
treatment within the package rate, fixed by the CGHS which includes the cost of
all the items.
14. If one or more minor procedures form part of a major treatment procedure,
then package charges would be permissible for major procedure and only 50% of
charges for minor procedure.
15. Any legal liability arising out of such services shall be the sole responsibility
and shall be dealt with by the concerned empanelled hospital. Services will be
provided by the Hospitals as per the terms given above.
16. Ministry of Health & Family Welfare reserves the right to withdraw/cancel
the above a.M. without assigning any reason.
17. The order takes effect from the date of issue of the a.M.
18. The authorities of Sundaram Arulrhaj Hospital, Tuticorin will have to enter
into an agreement with the Government of India to the effect that the Hospital will
charge from the Central Government employees at the rates fixed by the
Government and they will have to sign a Memorandum of Understanding (MaU)
(2 copies enclosed only for Hospital) within a period of 3 months from the date of
issue of the above mentioned OM failing which the Hospital will be derecognized.
Subject to above, the Hospital can start treating Central Government employees
covered under CS(MA) Rules, 1944. Q .

5. The undersigned is further directed to clarify as under.-
(a) "Package Rate" shall mean and include lump sum cost of in-patient
treatment//day care/diagnostic procedure for which a CS(MA) beneficiary has been I
permitted by the competent authority or for treatment under emergency from the •
time of admission to the time of discharge, including (but not limited to)-(i)
Registration charges, (ii) Admission charges, (iii) Accommodation charges
including patient's diet, (iv) Operation charges, (v) Injection charges, (vi) Dressing
charges, (vii) Doctor/consultant visit charges, (xi) Anesthesia chatges, (xii)
Operation theatre charges, (xiii) Procedural charges / Surgeon's fee, (xiv) Cost of
surgical disposables and all sundries used during hospitalization, (xv) Cost of
medicines, (xvi) Related routine and essential investigations, (xvii) Physiotherapy
charges etc, (xviii) Nursing care and charges for its services.
(c) Treatment charges for new born baby are separately reimbursable in addition
to delivery charges for mother.
(d) Hospitals I diagnostic centers empanelled under CS(MA) Rules, 1944 shall
not charge more than the package rates.
(e) Expenses on toiletries, cosmetics, telephone bills etc. are not reimbursable
and are not included in package rates.
(b) The package rates given in rate list are for semi-private ward.
(c) The package rates prescribed are for semi-private ward. If the beneficiary
is entitled for general ward there will be a decrease of 100/0 in the rates; for private
ward entitlement there wi II be an increase of 15%. However, the rates shall be
same for investigation irrespective of entitlement, whether the patient is admitted
or not and the test, per-se, does not require admission.
9. (a) The maximum room rent for different categories would be:

10. In case of non-emergencies, the beneficiary shall have the option of availing
specific treatmentlinvcstigation from any of the above mentioned hospitals of
his/her choice (provided the hospital is recognised for that treatment
procedure/test), after the specific treatment/investigation has been advised by
Authorised Medical Attendant and on production of valid ID card and permission
letter from his/her concerned Ministry/Department.
11. The recognised hospitals shall honor permission letter issued by competent
authority and provide treatment/investigation facilities as specified in the
permission letter.

15. If one or more minor procedures form part of a major treatment procedure,
then package charges would be permissible for major procedure and only 50% of
charges for minor procedure.
16. Any legal Iiability arising out of such services shall be the sole responsibility
and shall be dealt with by the concerned empanelled hospital. Services will be
provided by the Hospitals as per the terms given above.
17. Ministry of Health & Family Welfare reserves the righ~ to withdraw/cancel

The undersigned is directed to say that a number of representations from
various hospitals, Central Government Employees Welfare Coordination
Committees were received in the Ministry of Health & Family Welfare for
recognition of Private Hospitals at Gwalior (Madhya Pradesh) for treatment of
Central Government Employees and their family members under CS(MA) Rules,
1944.
(a) "Package Rate" shall mean and include lump sum cost of in-patient
treatment/lday care/diagnostic procedure for which a CS(MA) beneficiary has been
permitted by the competent authority or for treatment under emergency from the
time of admission to the time of discharge, including (but not limited to)-(i)

Registration charges, (ii) Admission charges, (iii) Accommodation charges
including patient's diet, (iv) Operation charges, (v) Injection charges, (vi) Dressing
charges, (vii) Doctor/consultant visit charges, (xi) Anesthesia charges, (xii)
Operation theatre charges, (xiii) Procedural charges / Surgeon's fee, (xiv) Cost of
surgical disposables and all sundries used during hospitalization, (xv) Cost of
medicines, (xvi) Related routine and essential investigations, (xvii) Physiotherapy
charges etc, (xviii) Nursing care and charges for its services.
(c) Treatment charges for new born baby are separately reimbursable in addition
to delivery charges for mother.
(d) Hospitals / diagnostic centers empanelled under CS(MA) Rules, 1944 shall
not charge more than the package rates.
(e) Expenses on toiletries, cosmetics, telephone bills etc. are not reimbursable
and are not included in package rates.
In case, there are no CGHS prescribed rates for any test/procedure, then AIIMS
rates are applicable. If there are no AIIMS rates, then reimbursement is to be
arrived at by calculating admissible amount item-wise/ e.g. room rent,
investigations, cost of medicines, procedure charges etc) as per approved
rates/actually, in case of investigations. ~

6. (a) CS(MA) beneficiaries are entitled to facilities of private, semi-private or
general ward depending on their basic pay. The entitlement is as follows:-

(b) The package rates given in rate list are for semi-private ward.
(c) The package rates prescribed are for semi-private ward. If the beneficiary
is entitled for general ward there will be a decrease of 10% in the rates; for private
ward entitlement there will be an increase of 15%. However, the rates shall be'
same for investigation irrespective of entitlement, whether the patient is admitted
or not and the test, per-se, does not require admission.
8. (a) The maximum room rent for different categories would be:
(b) Room rent mentioned above at (a) above is applicable only for treatment
procedures for which there is no CGHS prescribed package rate. Room rent will
include charges for occupation of Bed, diet for the patient, charges for water and
electricity supply, linen charges, nursing charges and routine up keeping.

as well as a bed for attendant. The room has to be air-conditioned.

18. The authorities of Kalyan Memorial & KDJ Hospital, Baradari Crossing,
Morar, Gwalior (Madhya Pradesh) will have to enter into an agreement with the
Government of India to the effect that the Hospital will charge from the Central
Government employees at the rates fixed by the Government and they will have to
sign a Memorandum of Understanding. (MOD) (2 copies enclosed only for
Hospital) within a period of 3 months from the date of issue of the above
mentioned OM failing which the Hospital will be derecognized. Subject to above,
the Hospital can start treating Central Government employees covered under
CS(MA) Rules, 1944.

powers to the Heads of Departments of Ministries / Departments to accord post-
facto approval/permission in cases where an employee or his dependent family
members go to an empanelled institution after due prescription by a Government
specialist but without obtaining prior permission from his / her Ministry / Department,
subject to the Head of Department being satisfied with the genuineness of the
reason which made the beneficiary go directly to an empanelled institute even

With reference to the above mcnt ioned matter the undersi uned is directed to state that

individual Plastic Cards are being issued to all ('('illS bcncficrar icx in Delhi and NC~J{ l ill place
of Family Cards. In this regard this ntTice has been receiving several queries seeking
clarifications and therefore ~ this IVl in istrv has decided to issue an Office Mernorand um
Clarifying the issues.
It is clarified that: "
1), Data of C'(JI-IS beneficiaries is available on Data base or Servers located at NIC'
Headq uartcrs.
2) The individual Plastic Cards arc unly Identity cards bearing a unique number for each
beneficiary. It is only an identity card for accessing the Data on Computer Data Base.
Address of beneficiary, validity for ('(iI1S facilities ~ na111C of wellness Centre are
available in Data base,
~). The Plastic' Cards are issued for a maximum period of Five years or till cntitllcd for
CGI·IS benefits ~ whichever mav he earlier. In case of CCllfS pensioners who have paid
for ~Rest of Life ~ faei lities anew plastic Cards shall be issued after Five years ~ without

any additional contribution. Similar!v , New Plastic Cards shall be issued to serving

6). In case of Pensioners applying for (~'(jlfS Cards applying for the first time in Delhi ~ an
acknowledgment slip is issued immediately on submission of complete Set of
Documents and prescribed subscription Icc. The print {Jut Slip is valid for avai ling CCiI-lS
facilities till Plastic Cards are issued. individual Plastic Cards are issued within -7' Days

8) Permissions for treatment shall be granted on the basis of Ben ID (Beneficiary ldentitv
Number) printed on Plastic Cards. While granting permission Data like the name of
Serving employee I Pensioner and the relationship of individual family member ctc.. can
be verified at hnp i//cghs.uic.ut/welcorne.j sp . Similarly. hospitals I Diagnostic Centres
can verify Data at http://cghs.llic.il\.\.velc\)me.jsp . ill case a beneficiary is admitted under
emergency.
9). 11,.tthe time of Submission of Medical Claim the Ben II) number of Serving employee or
Pensioner shall be entered on Modified Medical 2004 form as he I she shall he the
claimant. Copy of Plastic Card of Patient shall be enclosed along with Me(h:-:d
Reimbursement Claim.
10). In case of loss of Plastic Card, Serving employee! Pensioner shall apply at CGIIS(HQ)
for Duplicate Plastic Card along with IPO for Rs.50 !a copy of FIR lodged with POlice. a
copy of old Card. A 'print out slip' shall be issued immediately for availing C(jHS
facilities and Plastic Card aller 'T Days. In case of serving employees the application
shall be forwarded by the his / her department.
J I). In case of change in residential address and shifting from one dispensary to another,
CMO lie of Dispensary shall make modifications in Data base and transfer the cards to
new dispensary. CMO i/c of new dispensary shall accept 'transfer of Card and Data shall
be transferred to new dispensary
12). In case of superannuation I transfer to another city, serving employees get the cud
deleted from Data base at Dispensary and obtain a certificate from Ctvl0 i/c and
surrender the card to his Iher department. Department shall issue a surrender certificate to
employee for getting a new Card at another city ( if covered under CGHS) or for
obtaining a pensioner CGHS card in case of superannuation.
13). It is compulsory to bring original Plastic Cards every time for availing CGHS benefits.
14). All beneficiaries entitled (or semi-private ward in erupanelled private hospitals are
eligible for Nursing Home facilities in Govt. Hospitals and those entitled for Private
ward are eligible for Direct Consultation with specialists in Govt. Hospitals. Similarly,
beneficiaries entitled for Private ward in empanelled private hospitals are also eligihle for
Private ward facilities at A.I.I.I\'1.S .. New Delhi. No separate endorsement is required for
these cri teria.

b) Room rent is applicable only for treatment procedures for which there is
no CGHS prescribed package rate.
Room rent will include charges for occupation of Bed, diet for the patient,
charges for water and electricity supply, linen charges, nursing charges and
routine up keeping,.
15. Any legal liability arising out of such services shall be the sole
responsibility and shall be dealt with by the concerned empanelled hospital /
diagnostic centre. Services will be provided by the Hospitals/Diagnostic centers
as per the terms of Agreement.
16. The hospitals / diagnostic centres will not refuse permission to Centra!
Government employees or their dependent family members who are not CGHS
beneficiaries if they produce certified / attested copies of identity cards issued by
the Government of India and have been referred to the hospital I diagnostic
centre by the organisation in which they are working.
17. The hospitals / diagnostic centres will give a discount of 5% on every cash
payment charged directly from the beneficiaries.
19. A copy of this Office Memorandum along with copy of MOA is placed c:\
the internet at www.mohfw.nic.in/cghs.htm
