Chapter 8 Color Mode Image Optimization

8.1 Usage
Color Doppler flowmeter is the use of red blood cells and ultrasound Doppler effect between the
imaging. Color Doppler flowmeter includes 2D ultrasound imaging system, pulse Doppler
(one-dimensional Doppler) blood flow analysis system and color Doppler (two-dimensional Doppler)
blood flow imaging system. Since each sample contains a number of erythrocytes produced by the
Doppler flow information, so after autocorrelation detection is a number of blood flow velocity mixed
signal. The autocorrelation test results are sent to the speed calculator and the variance calculator to
obtain the average velocity, along with the FFT-processed blood flow spectrum information and the
2D image information stored in the digital scan converter. Finally, according to the direction of the
blood flow and the size of the color processor by the color data as a pseudo-color coding, color
display to display, to complete the color Doppler flow imaging. The color flow pattern is a Doppler
mode for adding color coded qualitative information related to the relative velocity and direction of
fluid motion in a B mode image.

8.2 Introduction
The use of color flow pattern check is generally carried out as follows:
1. The procedure described in the B mode is used to locate the specified anatomical region.
2. After optimizing the B mode image, add colored blood flow.
3. Move the color flow area to be viewed as close as possible to the center of the image.
4. Optimize the color flow parameters to achieve a higher frame rate and display the appropriate
blood flow velocity.
5. If you need more restrictive information about blood flow, use the procedure described in
Doppler mode.

Chapter 9 Optimize Color Doppler Images

9.1 Usage
Doppler provides measurement data on the velocity of the moving tissue and fluid. PW (Color)
Doppler allows you to selectively check blood flow data from a small area (ie, sampling volume).

9.2 Typical applications - PW (Pulse Wave) Doppler

In Pulsed Wave Doppler (PW) mode, energy is emitted from the ultrasound probe into the
patient, which is the same as in B mode. However, the received echo will be processed to find the
difference between the transmit and receive signal frequencies. The difference between frequencies
may be caused by the movement of the ultrasound signal through moving objects in the path, such as
blood cells. The resulting signal will be represented by the system speaker in audio mode and
displayed graphically on the system display. The X-axis in the figure represents the time and the
Y-axis represents the frequency shift. The Y axis can also represent the forward or reverse speed after
calibration. A typical application of PW Doppler is to display the velocity, direction, and spectral
content of the blood flow in the selected anatomical region. PW Doppler can be used in conjunction
with the B mode to quickly select the anatomical area for PW Doppler examination. The part from
which the PW Doppler data is obtained is displayed graphically in the B-mode image (sample volume
gate). The sampling volume door can be moved freely in the B mode image.

9.3 Typical inspection program

The use of PW Doppler mode check is generally carried out as follows:
1. Connect the appropriate probe and place the probe in the appropriate holder.
2. Allow the patient to be in the proper position for inspection.
3. Press Patient. Enter the appropriate patient data in the appropriate check category.
4. Select the preset, application and probe.
5. Locate the anatomy to be examined. Get a higher quality B-mode image.
6. Press "PW" knob, sample line, sampling volume line and the sampling volume gate show on
display. Then press Updata. The PW Doppler spectrum is displayed on the screen and the
system will operate in the B + C + D combination mode. Adjust the "PW" knob or the "C"
knob to adjust Doppler audio. The Doppler signal can be heard through the speaker.
7. Position the sampling volume line by moving the trackball left and right. Position the
sampling volume gate by moving the trackball up and down. Change the sampling volume
gate size by pressing the "Set" key to select the sampling width (sample volume length).
8. If necessary, optimize the PW Doppler spectrum. For more information, see the "Doppler

- 67 -
Color Ultrasonic Diagnostic Apparatus User Manual

Optimization" section of this chapter.

9. Press "PW" knob to switch between real-time B mode and Doppler mode.
10. Samples along the length of the entire vessel to ensure that the probe is parallel to the
direction of the blood flow; when positioning the sampling volume line, listen again.
11. Press “Freeze” to freeze the current image for image storage and cine loop.
12. If necessary, perform measurements and calculations. For more information, see the
"Measurement and Calculation" chapter.
13. The result is recorded by pressing the appropriate print key, depending on the settings of the
recording device.
14. Repeat the above steps until all relevant blood flow parts have been checked.
15. Place the probe back into the corresponding holder.

Activate Triplex mode

To activate the Triplex mode, press "C" knob, then press "PW" knob. Select "Triplex" option on

the left side of the screen and press “ ” or “ ” key to turn on the Triplex mode. The Doppler
spectrum is displayed along with the color flow and B mode images. The cursor changes to a Doppler
sampling line. You can now determine the position and size of the sampling volume gate to get a
certain speed. Use Doppler audio to determine whether the sampling volume gate is located in the
blood flow area. Use the Triplex mode to check blood flow information. To exit the PW Doppler mode,
press the “PW” knob, and then press the “B” knob.

9.4 Spectrum Doppler display

Description
Time zero (the starting point of the track) appears on the left side of the graph. As time goes by,
the track moves to the right. The baseline of the graph (representing zero speed, zero frequency shift,
or no blood flow detected) is displayed as a solid line across the screen. By convention, the movement
toward the probe direction is positive and the movement away from the probe direction is negative.
Positive frequency or velocity appears above the baseline. A negative frequency or velocity appears
below the baseline. In general, blood flow is not uniform, but by different speed and movement in
different directions of blood cells composed of mixed. Thus, the displayed content is a combination of
spectra with different grayscale values. The strong signal shows brighter, while the weak signals are
displayed in varying degrees of gray.

10.5.1 Abdomen, General Measurement
Description
The human abdomen is the body part between the pelvis and the chest. In the anatomy, the
abdomen from the chest to the diaphragm of the true and false pelvic boundaries. The true and false
pelvic boundaries from the lumbosacral angle (the fifth lumbar vertebrae and the first sacral vertebrae
between the discs) to the pubic symphysis, the edge of the pelvic entrance. The cavity between the
pelvic inlet and the diaphragm is called the abdominal cavity. The peritoneal border is the abdominal
wall. In function, the abdomen is where most of the digestive tract, which means that digestion and
absorption are occurring here. The digestive tract in the abdominal cavity includes the lower
esophagus, stomach, duodenum, jejunum, ileum, cecum and appendix, ascending colon, transverse
colon and descending colon, sigmoid colon and rectum. Other important organs are liver, kidney,
pancreas and spleen The abdominal wall is divided into the posterior side and the anterior abdominal
wall.
All operations under abdominal measurements must be performed in freeze state.

10.6.4 Obstetrics Description

Measurements and calculations obtained from ultrasound images should be used as a supplement
to other clinical measures of the physician. The accuracy of the measurement of the system is not
only affected by the accuracy of the system itself, but also depends on whether the user has
adopted the correct medical program. If necessary, be sure to indicate a medical plan related to
measurement and calculation. This section begins with all representative B-mode measurements
included in the obstetric study. Other obstetric measurements follow these representative
measurements.

method circumference / area measurement; AFI measurement need four distances.

10.6.6 Calculate FW

Users can also measure AC/BPD, AC/FL or AC/FL/HC. The data will be automatically stored in
memory. When the obstetric report is called, the system will automatically calculate the result
according to the multi-parameter calculation.

10.6.7 Calculate GA and EDC by LMP

The calculation method of the EDC, in the previous section has been based on obstetric
measurement automatically calculate the GA and EDC, here to introduce to calculate GA and EDC by
LMP. As a means of expression during pregnancy, from the occurrence of the point of view, is the
fertilization day as the first day, the whole pregnancy of 280 days, 10 months of gestation (28 days
each pregnent month).
Because every pregnant woman is difficult to accurately determine the time of conception, in the
clinical application of obstetrics, is the date of the last menstrual start as zero weeks of pregnancy to
calculate the first 40 weeks of the zero day is the expected date of accurate date. Pregnant women in
the 38-42 weeks of pregnancy delivery, are full-term delivery. This algorithm is the 28-day type as a
standard, because each woman's menstrual cycle length varies, so the expected pre-production period
and the actual expected date of 1-2 weeks of access is normal.
<The calculation method by LMP>
EDC(LMP)=LMP+280(days)
(LMP:short for Last Menstrual Period, the first day of the last menstruation)

10.6.9 Fetal Biophysical Score

Fetal Biophysical Score: there are two commonly used scoring methods:
Mannig scoring method——Will each of the physiological variables, the normal assessment of 2
points; abnormal comment 0 points. To observe the followings in 30 minutes : 1. fetal respiratory
movement; 2. large body movement; 3. fetal muscle tension; 4. reactive FHR; 5. amniotic fluid
quantitative.
Vintzileos scoring method——Each physiological activity was scored as "0", "1", "2".
1. To observe the reactive FHR in 20 minutes ; to observed the followings in 30 minutes: 2.
fetal movement; 3. fetal respiratory movement; 4. fetal muscle tension; 5. amniotic fluid; 6. placental
maturity.
In medical practice, the newborns are widely used Apgar scoring method named by Apgar to judge
this, including the following five projects:
1.Skin color: assess the situation of neonatal lung oxygen exchange. Body skin was pink for 2
points, hands and feet were purple purple for 1 minute, the body was purple for 0 points.
2.Heart rate: assess the intensity and rhythm of neonatal heart beats. Heart beat more than 100
times / minute for 2 points, heart beat less than 100 times / minute for 1 point, can not hear the heart
sound for 0 points.
3 . Breath: assess the maturity of the neonatal center and lungs. Breathing law for 2 points,
breathing rhythm irregular (such as shallow or irregular or rapid effort) for 1 point, no breathing for 0
points.
4.Muscle tension and exercise: assessment of neonatal central reflex and muscle strength. Muscle
tension is normal for 2 points, abnormal muscle tension or low for 1 point, muscle tension relaxation
of 0 points.
5. Reflex: assess the ability of the newborn to respond to external stimuli. On the shell foot or
other stimuli loud cry for 2 points, whispered sobbing or frown for 1 point, no response to 0 points.
According to these five signs as the basis, full of 10 points were normal neonatal, score 7 points
below the neonatal consider suffering from mild asphyxia, score below 4 points to consider suffering

- 100 -
Color Ultrasonic Diagnostic Apparatus User Manual

from severe asphyxia. Most of the newborns score among 7-10 points, the doctor will be based on the
child's score to be dealt with accordingly. Mild asphyxia neonates generally cleaned up the respiratory
tract, oxygen and other measures will soon improve, the prognosis is good.
Clinical significance: usually considered the level of fetal score and neonatal Apgra score, fetal
distress during childbirth, prenatal and perinatal mortality are highly correlated. The lower the score,
the higher the correlation between the two. It is reported that when all scoring items are normal, the
fetal physiological score is "10", the perinatal mortality rate is 0; the other hand, if all parameters are
abnormal, fetal physiological score of "0", the fetal mortality rate of 40%.
The system uses the Vintzileos scoring method to rate the following items:
Reactive fetal heart rate accelerate Fetal movement Fetal respiratory activity Fetal
muscle tension AFI

Operation steps:
Move the trackball, select the measurement page of the obstetric report, select page 3, and the
report menu shows the above functions, and the corresponding selection is made.

10.12 Cardiac Measurement

10.12.1 Description
Heart is in the chest, the top of the diaphragm, between the two lungs, about two-thirds on the
left side of the midline. The heart is like an inverted, front and rear slightly flat cone. Apex obtuse,
toward the left front and lower, and the chest wall adjacent to its surface projection in the left chest
anterior wall of the fifth intercostal space clavicle medial 1-2cm, so here can be seen or touch the apex
beat. The bottom of the heart is wide, large blood vessels from this access, toward the right after the
top, and esophageal and other posterior mediastinal organs adjacent. The heart wall is made of
myocardium and can automatically contract at about 70 rhythms per minute, thereby continuously
transporting blood to whole body tissues. The heart is actually made of two blood pumps. Each blood
pump is made up of two parts: the upper part is called the atrium; the lower part is called the ventricle.
Blood enters the heart from the atria and flows into the lower ventricle. The ventricular wall is very
hypertrophy and helps to pump the blood out of the heart. Heart work, the right ventricle to send
blood to the lungs, blood flow through the lungs back to the left atrium, and then transported to the
body through the left ventricle.

Description
The left ventricle wall of the heart is thicker than the right ventricle wall, which is adapted to the
iterative (aorta), into the (body) cycle. Human venous blood is from the body back to the heart of the
blood back to the right atrium right ventricle and then into the lungs through the alveolar capillaries
into arterial blood back to the left atrium left ventricle in the body to the body.

10.12.5 RVWd Measurement

Description
The thickness of the atrial wall is related to the distance between the delivery of blood. Compared

with the atria, the wall thickness of some of the heart, because the heart of the blood flow only with its
ventricle; left ventricular wall thicker than the right ventricle. The left ventricle blood to the body, so
the need for greater contractility, the wall will be thicker. The right ventricle to the blood delivery to
its very close to the lungs, the need for some smaller shrinkage, the wall is thinner. Normal, the right
ventricular wall thickness of only 1/3 of the left ventricular wall.

10.12.7 RVWs Measurement

Description
Ventricular wall stress is an important indicator of cardiac function and myocardial oxygen supply,
and it is important to accurately estimate changes in stress in the wall. The traditional method of
estimating ventricular wall stress is measured by 2D echocardiography or cinematic angiography, and
only the mean value can only be estimated. However, ventricular wall stress is a local functional
parameters, due to the complexity of the ventricular wall geometry and its high degree of non-linear
characteristics, in vitro myocardial studies can not be directly applied to the conclusions of the living
heart, people put forward a number of physical prediction of ventricular wall stress mathematical
model, wall stress model proposed by Arts and other people is more comprehensive, the ventricular
contraction around its long axis of rotation and myocardial anisotropy are taken into account, the
formula is as follows: wall stress = ESP × (3ESV / WV +1).
Note: ESP is for the left ventricular end-systolic pressure, ESV is for the end of the left ventricular
volume, WV for the wall volume. The left ventricular systolic end of the local end-systolic stress
study found that from the apical to the bottom of the heart wall stress gradually increased, the study
also found that patients with dilated cardiomyopathy and end-systolic left ventricular wall stress was
significantly higher than normal.
