You have not attended work since [insert date] and have failed to respond to our previous letter
dated [insert date], requesting that you contact us to discuss your absence and the circumstances
surrounding your failure to notify. As you have failed to do that, we have no alternative other than
to assume you do not intend to return and have abandoned your employment. Therefore we have
take action to terminate your employment.
