NOTICE TO EXPLAIN

RE: INSUBORDINATION/NEGLECT OF DUTY

In several occasions from March 2020 to present you had failed to coordinate with your
immediate supervisor regarding urgent office matters particularly the updating,
processing and payment of utility and communications billings which is part of your
duties and responsibilities. Due to this negligence, most of our office communication
lines and hose assigned to key employees was cut due to non-payment.

You were contacted by your immediate supervisor through mobile phone (call and text),
FB messenger, viber messages and email, to inquire and/or update said billings but you
refused to answer her queries. Your negligence and failure to follow the directives of
your immediate superior without any valid/justifying circumstance was detrimental to the
interest of the company. to warrant your absence. These act can be considered to be
disobedience, misconduct and gross negligence of your duties that goes against our
company’s employee attendance policy and the Labor Code of the Philippines as
amended.

YOU ARE HEREBY DIRECTED BY THIS OFFICE TO SUBMIT your written explanation
within Five (5) days from receipt of this notice why you should not be meted with the
appropriate sanction/penalty for your negligence and insubordination. Non-response on
your part shall be considered as a waiver of your right to explain and contest the
appropriate administrative charges against you and the corresponding penalty thereof.
