The aforementioned acts and omissions charged against you constitute AWOL under our company policies
and procedure.
As per checking, this happen to be your 3rd offense with a corresponding penalty of third (30) days
suspension.
You are hereby directed to explain in writing within six (6) days from receipt of this notice why you should
not be penalized / disciplinarily dealt with the appropriate penalty for the above mentioned charge.
Failure to submit your written explanation shall be deemed a waiver on your part and we shall be
constrained to take the necessary action based on evidence on record.
Be guided accordingly.
