You were expected to be on duty as indicated in your manning schedule; however, you failed to report to
work on the date/s, to wit:

March 25 and 26, 2019

The aforementioned acts and omissions charged against you constitute AWOL under our company policies
and procedure.

As per checking, this happen to be your 3rd offense with a corresponding penalty of third (30) days
suspension.

You are hereby directed to explain in writing within six (6) days from receipt of this notice why you should
not be penalized / disciplinarily dealt with the appropriate penalty for the above mentioned charge.
Failure to submit your written explanation shall be deemed a waiver on your part and we shall be
constrained to take the necessary action based on evidence on record.

Be guided accordingly.
