1. Data analysis
1. Background
Hypertension is a state of increased systolic blood pressure of at least
140 mmHg or a diastolic pressure of slightly 90 mmHg. Hypertension is not
only at high risk of suffering from heart disease, but also at risk of suffering
from other diseases such as nerve, kidney and blood vessel diseases and the
higher the blood pressure, the greater the risk ( Sylvia A. Price in NANDA
NIC-NOC, 2015). Hypertension is a major cause of heart failure, stroke and
kidney failure. High blood pressure is called a silent killer, because people
with high blood pressure often do not show symptoms.
The National Institute of Heart, Lungs and Blood estimates that half
of the people who suffer from high blood pressure are unaware of their
condition. As soon as this disease suffers and causes symptoms, the patient's
blood pressure must be monitored at regular intervals because high blood
pressure is a lifelong condition.
So based on the results of the assessment on the family of Mr. M
obtained one of the diagnoses of family nursing that has a definition
of knowledge, the family does not understand the current conditions related to
both the cause and what are the risks going forward. Therefore , students make
plans to provide an understanding related to the conditions experienced by Mr.
M's family today with the aim that the family is able to take care of the
conditions experienced at this time and not cause undesirable conditions in the
future.
2. The theme :"Efforts to improve knowledge of the family Mr. M in the case
of hypertension by conducting health education in the village of Tanjung
Tengah regency,CentralLombok.

After doing health education for 1x25 minutes it is expected that the
family of Mr. M can understand about hip ertensi and how to prevent it,
and be able to perform self-care to high blood pressure.
b) Special Instructional Purpose (ICT)
After being given counseling by the family of Mr. M can:
1) Mr. M can understand what is the meaning of hypertension
2) Mr. M can understand the causes of hypertension
3) Mr. M can understand what are the signs and symptoms of
hypertension
4) Mr. M understands how to prevent hypertension so that it does not get
worse.
5) Family Tn.M can understand what any complications will happen to the
current condition.
6. Main Material for Hypertension
a) Definition of hypertension
Hypertension is as an increase in systolic blood pressure of at
least 140 mmHg or a diastolic pressure of slightly 90
mmHg. Hypertension is not only at high risk of suffering from heart
disease, but also at risk of suffering from other diseases such as nerve,
kidney and blood vessel diseases and the higher the blood pressure, the
greater the risk ( Sylvia A. Price in NANDA NIC-NOC, 2015).
Hypertension is a condition that causes high blood pressure
continuously where systolic pressure is more than 140 mmHg, diastolic
pressure 90 mmHg or more. Hypertension or high blood pressure is a
condition of chronic increased blood circulation. This happens because the
heart works faster to pump blood to meet the needs of oxygen and
nutrients in the body (Koes Irianto, 2014).
b) Etiology
Hypertension based on the cause can be divided into two major groups,
namely: ( NANDA NIC-NOC , 2015)
1) Primary hypertension (essential)
Also called idiopathic hypertension because the cause is
unknown. Factors that influence it are: genetic, environmental,
hyperactivity, sympathetic nervous system renin. Angiotensin and
increased intracellular Na + Ca. Factors that increase risk: obesity,
smoking, alcohol and polistemia.
2) Secondary hypertension
The causes are: use of estrogen, kidney disease, cushing's syndrome
and hypertension related to pregnancy.
Hypertension based on advanced age can be divided into:
1) Hypertension where systolic pressure is equal to or greater than 140
mmHg and / or diastolic pressure is equal to or greater than 90
mmHg.
2) Isolated systolic hypertension where systolic pressure is greater than
160 mmHg and diastolic pressure is lower than 90 mmHg.
The cause of hypertension in people with elderly is the occurrence
of changes in:
1) Aurta's dinting elasticity decreases
2) The heart valve thickens and stiffens
3) The ability of the heart to pump blood decreases by 1% every year after
the age of 20 years, the ability of the heart to pump blood decreases
causing the decrease in contractions and volume.
4) Loss of elasticity of blood vessels. This happens because of the lack of
effectiveness of peripheral blood vessels for oxygenation
5) Increased peripheral vascular resistance
Clinically the degree of hypertension can be grouped namely:

c) Factors that influence hypertension
Risk factors that cannot be controlled :
1) Gender
The prevalence of hypertension in men and women. Women are
known to have lower blood pressure than men when they are 20-30
years old. But it will be easy to attack in women when they are 55
years old, about 60% suffer from hypertension affecting women. This
is associated with hormonal changes in women after menopause
(Endang Triyanto, 2014).
2) Age
Changes in blood pressure in a person will change stably at the age of
20-40 years. After that it will tend to increase more quickly. Thus, the
more a person ages, the more blood pressure rises. So an elderly
person tends to have higher blood pressure than young people (Endang
Triyanto, 2014).
3) Heredity (genetic)
The existence of genetic factors will certainly affect families who have
suffered from hypertension before. This occurs an increase
in intracellular sodium levels and a low ratio between potassium
to individual sodium so that the elderly tend to be at higher risk of
suffering from hypertension two times greater compared to people who
do not have a family history of hypertension (Buckman, 2010).
4) Education
The level of education indirectly affects blood pressure. The high risk
of hypertension in low education, the possibility of lack of knowledge
in receiving information by health workers so that it affects the
behavior or healthy lifestyle (Armilawaty, Amalia H, Amirudin R.,
2007).
Risk factors for hypertension that can be controlled:
1) Obesity
In middle age and old age, there tends to be a lack of activity so that
calorie intake balances energy needs, so that there will be an increase
in body weight or obesity and will worsen the condition (Anggara,
FHD, & N. Prayitno, 2013).
2) Lack of exercise
If you do regular exercise it will be easy to reduce the increase in high
blood pressure which will reduce peripheral resistance, so that you
train your heart muscles to get used to doing heavier work because of
certain conditions.
3) Smoking habits
Smoking can increase blood pressure . This is because in the nicotic
content that can cause narrowing of blood vessels .
4) Excessive salt consumption
WHO recommends consuming salt which can reduce the increase in
hypertension. Levels of sodium that is recommended is not more than
100 mmol (about 2.4 grams of sodium or 6 grams) (H. Hadi Martono
Kris Pranaka, 2014-2015).
5) Drink alcohol
When consuming excessive alcohol will cause an increase in blood
pressure that is classified as severe because it can cause blood in the
brain to become blocked and cause a stroke.
6) Drink coffee
One cup of coffee contains 75-200 mg of caffeine, which in one cup of
coffee can increase blood pressure by 5-10 mmhg.
7) Anxiety
Anxiety will cause a sympathetic stimulus that will increase heart rate,
cardiac output and vascular resistance, these side effects will increase
blood pressure. Anxiety or stress increases blood pressure by 30
mmHg. If the individual feels anxious about the problem at hand,
hypertension will occur in him. This is because anxiety that will
repeatedly affect the heart rate faster so that the heart pumps blood
throughout the body will be faster.
d) Signs and symptoms
According to Amin Huda Nuraif & Hardi Kusuma, 2015 Signs and
symptoms in hypertension can be divided into:
1) There are no symptoms
There are no specific symptoms that can be associated with an increase
in blood pressure, other than the determination of arterial pressure by
the examining doctor. This means that arterial hypertension will never
be diagnosed if arterial pressure is not regular.
2) Common symptoms
It is often said that the most common symptoms that accompany
hypertension include headache, neck stiffness and fatigue. In reality
this is the most common symptom regarding most patients seeking
medical help . Some patients suffering from hypertension are;
a) Complaining of headaches and dizziness
b) Limp, tiredness
c) Nausea
d) Vomiting
e) Shortness of breath
f) S ulit sleep
g) Restless
h) Epistaxis
i) Declining awareness
Some patients who suffer from hypertension, namely:
complain of headaches, dizziness, fatigue, shortness of breath,
anxiety, nausea, vomiting, epistaxis and decreased consciousness.
e) Prevention
1) Primary prevention
a) Increase knowledge and education about the dangers of
hypertension
b) Implement and enhance healthy living behaviors
(1) Eat enough vegetables and fruit
(2) Low in salt and fat
(3) Do not smoke and do not consume alcohol
c) Get plenty of rest and exercise
d) Avoid activities that cause stress
e) Recognize other diseases that cause hypertension
2) Secondary Prevention
a) Periodic checks
(1) Measurement of blood pressure
(2) Control tension regularly to keep it stable
b) Treatment / care
(1) Immediate treatment
(2) Avoiding complications
(3) Stabilizing blood pressure
(4) Minimize side effects of treatment
(5) Avoiding risk factors for hypertension
(6) Treat comorbidities, such as: DM, CHD, etc.
3) Tertiary Prevention
a) Lowering blood pressure to normal level
b) Prevent high blood pressure so that it does not cause damage to
body tissues
c) Restoring organ damage with antihypertensive drugs
d) Control blood pressure so that it does not cause complications of
diseases such as stroke, CHD etc.
e) Perform proper and fast treatment, avoid disability and death due to
uncontrolled hypertension
f) Discharge planning
The dismissal planning that can be done to prevent hypertension are:
1) Stop smoking
2) Maintain a healthy lifestyle
3) Learn to relax and control stress
4) Limit alcohol consumption
5) If you already use hypertension medication, continue to use it
routinely
6) Salt diet and weight control
7) Check blood pressure regularly
g) Complications
According to Padila, 2013 High blood pressure if not treated and treated,
then in the long run will cause damage to the arteries in the body of organs
that get blood supply from these arteries. Complications of hypertension
can occur in the following organs:
1) Heart
High blood pressure can cause heart failure and coronary heart
disease. In patients with hypertension, the workload of the heart will
increase, the heart muscle will relax and decrease its elasticity, which
is called decompensation. As a result, the heart is no longer able to
pump so that a lot of fluid is retained in the lungs and other body
tissues which can cause shortness of breath or edema. This condition is
called heart failure.
2) Brain
Complications of hypertension in the brain, pose a risk of stroke, if left
untreated, the risk of stroke is 7 times greater.
3) Kidney
High blood pressure also causes kidney damage, high blood pressure
can cause damage to the filtering system in the kidneys as a result the
kidneys are unable to get rid of substances that are not needed by the
body that enter through the bloodstream and buildup occurs in the
body.
4) Eyes
Hypertension in the eye can cause hypertension retinopathy and can
cause blindness (Yahya, 2005).
h) Management
1) Management of Nonfarmacology
Nonpharmacological management with lifestyle modification is very
important in preventing high blood pressure and is an inseparable part
in treating high blood pressure (Ridwan amir uddin, 2007).
Management of hypertension with non-pharmacological consists of
various ways of lifestyle modification to reduce blood pressure,
namely:
a) Maintain ideal body weight
Maintain ideal body weight according to body mass index (BMI)
with a range of 18.5-24.9 kg / m2 (Kaplan, 2006). BMI can be
determined by dividing your weight that has been squared in a low
cholesterol diet but rich in fiber and protein (pfizerpeduli.com),
and if you succeed in losing weight 2.5-5 kg then diastolic blood
pressure can be reduced by 5 mmHg (Radmarssy, 2007).
b) Reduce sodium intake (sodium)
Reducing sodium intake can be done by a low salt diet that is no
more than 100 mmol / day (approximately 6 grams of NaCL or 2.4
grams of salt / day) b (Kaplan, 2006). Another amount by reducing
salt intake to less than 2300 mg (1 teaspoon) every day. The
regulation of salt consumption to ½ teaspoon / day can reduce
systolic pressure by 5 mmHg and diastolic pressure by around 2.5
mmHg (Radmarssy, 2007).
c) Limit alcohol consumption
Radmarssy (2007) said that consuming alcohol must be limited
because excessive alcohol consumption can increase blood
pressure. Heavy drinkers have a risk of hypertension four times
greater than those who do not drink alcoholic beverages.
d) Eat enough K and Ca from the diet
Maintain a diet of pottasium (> 90 mmol (3500 mg) / day) by
consuming high fruits and vegetables and a low-fat diet by
reducing the intake of saturated fat and total fat (Kaplan,
2006). Potassium can reduce blood pressure by increasing the
amount of sodium that is wasted with urine. By at least consuming
fruits 3-5 times a day, one can achieve adequate potassium intake
(Radmarssy, 2007).
e) Avoid smoking
Smoking is not directly related to the onset of hypertension, but
smoking can increase the risk of complications in hypertensive
patients such as heart disease and stroke, it is necessary to avoid
consuming tobacco (cigarettes) because it can aggravate
hypertension (Dalimartha, 2008).
The nicotine in tobacco makes the heart work harder
because it narrows blood vessels and increases the frequency of
heart rate and blood pressure (Sheps, 2005). So in patients with
hypertension it is recommended to stop smoking
(pfizerpeduli.com).
f) Reduction of stress
Stress does not cause permanent hypertension, but if episodes of
stress occur frequently, it can cause a very high temporary increase
(Sheps, 2005). Avoiding stress by creating a pleasant atmosphere
for people with hypertension and introducing various methods of
relaxation such as yoga or meditation that can control the nervous
system which can ultimately reduce blood pressure
(pfizerpeduli.com).
g) Massage therapy (massage)
According to Dhalimartha (2008), in principle massage done to
hypertension sufferers is to facilitate the flow of energy in the body
so that hypertension disorders and complications can be
minimized, when all energy pathways are open and energy flow is
no longer obstructed by muscle tension and other obstacles the risk
of hypertension can be pressed.
2) Pharmacological Medicine
a) Diuretics (Hydrochlorothiazides)
Removing body fluids so that the volume of body fluids is reduced
which results in a lighter heart pumping power.
(1) Sympathetic inhibition (Methyldopa, Clonidine, and
Reserpine)
Inhibiting sympathetic nerve activity.
(2) Betabloker (Metoprolol, Propanolol and Atenolol)
(a) Decreases the heart pump power.
(b) Not recommended for patients who are known to have
respiratory problems such as bronchial acid.
(c) In people with diabetes mellitus: can mask the symptoms of
hypoglycemia.
(3) Vasodilators (Prasosin, Hidralasin)
Works directly on blood vessels by relaxing the blood vessels
muscles.
(4) ACE inhibitors (Captopril)
(a) Inhibits the formation of Angiotensin II substances.
(b) Side effects: dry cough, dizziness, headache and weakness.
(5) Angiotensin II (Valsartan) Receptor Inhibitors
Blocking the attachment of Angiotensin II substances to the
receptors so that it can reduce the heart's pumping power.
(6) Calcium antagonists (Diltiasem and Verapamil)
Inhibiting heart contractions (contractility).

a. Defisit pengetahuan berhubungan dengan ketidakmampuan mengenal
masalah kesehatan (hipertensi) ditandai dengan ketidaktahuan
menemukan sumber informasi.
Untuk kasus ini kita memberikan intervensi penyuluhan
SATUAN ACARA PENYULUHAN
1. Latar Belakang
Hipertensi merupakan keadaan peningkatan tekanan darah sistolik
sedikitnya 140 mmHg atau tekanan diastoliknya sedikit 90 mmHg. Hipertensi
tidak hanya beresiko tinggi menderita penyakit jantung, tetapi juga beresiko
menderita penyakit lain seperti penyakit saraf, ginjal dan pembuluh darah dan
makin tinggi tekanan darah, maka makin besar resikonya (Sylvia A. Price
dalam NANDA NIC-NOC, 2015). Hipertensi merupakan penyebab utama
gagal jantung, stroke dan gagal ginjal. Tekanan darah tinggi disebut sebagai
pembunuh diam-diam, karena orang dengan tekanan darah tinggi sering tidak
menampakkan gejala.
Institut Nasional Jantung, paru dan darah memperkirakan separuh separuh
orang yang menderita tekanan darah tinggi tidak sadar akan kondisinya.
Begitupenyakit ini diderita dan menimbulkan tanda gejala , tekanan darah
pasien harus dipantau dengan interval teratur karena darah tinggi merupakan
kondisi seumur hidup.
Sehingga berdasarkan hasil pengkajian pada keluarga Tn. M didapatkan
salah satu diagnosa keperawatan keluarga mengalami defisit pengetahuan,
keluarga belum memahami terkait kondisi yang dialaminya saat ini baik dari
penyebab dan apa resiko kedepannya. Oleh karena itu maka mahasiswa
membuat rencana untuk memberikan pemahaman terkait kondisi yang dialami
keluarga Tn.M saat ini dengan tujuan agar keluarga mampu melakukan
perawatan terhadap kondisi yang dialami saat ini serta tidak menyebabkan
kondisi yang tidak diinginkan kedepannya.
2. Tema : “ Upaya peningkatan pengetahuan keluarga Tn. M pada
kasus hipertensi dengan melakukan penyuluhan kesehatan di desa tanjung
kabupaten lombok tengah”.

4. Strategi Pelaksanaan

5. Tujuan
a. Tujuan Intruksional Umum (TUM)
Setelah dilakukan penyuluhan kesehatan selama 1x25 menit diharapkan
keluarga Tn. M dapat memahami tentang hipertensi dan cara
pencegahannya, serta mampu melakukan perawatan diri terhadap penyakit
darah tinggi.
b. Tujuan Intruksional Khusus (TIK)
Setelah diberikan penyuluhan keluarga Tn. M dapat :
1) Keluarga Tn. M dapat memahami apa pengertian hipertensi
2) Keluarga Tn. M dapat mengerti penyebab hipertensi
3) Keluarga Tn. M dapat mengerti apa saja tanda dan gejala hipertensi
4) Keluarga Tn. M mengerti bagaimana cara pencegahan hipertensi agar
tidak menjadi lebih parah.
5) Keluarga Tn.M dapat memahami apa saja komplikasi yang akan terjadi
terhadap kondisinya saat ini.
6. Pokok Materi Hipertensi
a. Pengertian hipertensi
Hipertensi adalah sebagai peningkatan tekanan darah sistolik
sedikitnya 140 mmHg atau tekanan diastoliknya sedikit 90 mmHg.
Hipertensi tidak hanya beresiko tinggi menderita penyakit jantung, tetapi
juga beresiko menderita penyakit lain seperti penyakit saraf, ginjal dan
pembuluh darah dan makin tinggi tekanan darah, maka makin besar
resikonya (Sylvia A. Price dalam NANDA NIC-NOC, 2015).
Hipertensi merupakan suatu keadaan yang menyebabkan tekanan
darah tinggi secara terus-menerus dimana tekanan sistolik lebih dari 140
mmHg, tekanan diastolik 90 mmHg atau lebih. Hipertensi atau penyakit
darah tinggi merupakan suatu keadaan peredaran darah meningkat secara
kronis. Hal ini terjadi karena jantung bekerja lebih cepat memompa darah
untuk memenuhi kebutuhan oksigen dan nutrisi di dalam tubuh (Koes
Irianto, 2014).
b. Etiologi
Hipertensi berdasarkan penyebabnya dapat dibedakan menjadi dua
golongan besar yaitu: (NANDA NIC-NOC, 2015)
1) Hipertensi primer (esensial)
Disebut juga hipertensi idiopatik karena tidak diketahui penyebabnya.
Faktor yang mempengaruhinya yaitu: genetik, lingkungan,
hiperaktifitas, saraf simpatis sistem renin. Angiotensin dan
peningkatan Na + Ca intraseluler. Faktor-faktor yang meningkatkan
resiko: obesitas, merokok, alkohol dan polistemia.
2) Hipertensi sekunder
Penyebab yaitu: penggunaan estrogen, penyakit ginjal, sindrom
cushing dan hipertensi yang berhubungan dengan kehamilan.
Hipertensi berdasarkan usia lanjut dibedakan atas:
1) Hipertensi dimana tekanan sistolik sama atau lebih besar dari 140
mmHg dan/ atau tekanan diastoliknya sama atau lebih besar dari 90
mmHg.
2) Hipertensi sistolik terisolasi dimana tekanan sistolik lebih besar dari
160 mmHg dan tekanan diastolik lebih rendah dari 90 mmHg.
Penyebab hipertensi pada orang dengan lanjut usia adalah
terjadinya perubahan-perubahan pada:
1) Elastisitas dinting aurta menurun
2) Katub jantung menebal dan menjadi kaku
3) Kemampuan jantung memompa darah menurun 1% setiap tahun
sesudah berumur 20 tahun kemampuan jantung memompa darah
menurun menyebabkan menurunya konteraksi dan volumenya.
4) Kehilangan elastisitas pembuluh darah. Hal ini terjadi karena
kurangnya efektifitas pembuluh darah perifer untuk oksigenasi
5) Meningkatnya resistensi pembuluh darah perifer
Secara klinis derajat hipertensi dapat dikelompokkan yaitu: (Amin
Huda Nuraif & Hardi Kusuma, 2015).

3) Pencegahan Tersier
a) Menurunkan tekanan darah ketingkat normal
b) Mencegah memberatnya tekanan darah tinggi sehingga tidak
menimbulkan kerusakan pada jaringan tubuh
c) Memulihkan kerusakan organ dengan obat anthipertensi
d) Mengontrol tekanan darah sehingga tidak menimbulkan komplikasi
penyakit seperti stroke, PJK dll
e) Melakukan penanganan tepat dan cepat, menghindari kecacatan
dan kematian akibat hipertensi tak terkendali
f. Discharge planning
Adapun perencanaan pemberhentian yang dapat dilakukan untuk
mencegah hipertensi adalah :
1) Behenti merokok
2) Pertahankan gaya hidup sehat
3) Belajar untuk rileks dan mengendalikan stress
4) Batasi konsumsi alkohol
5) Jika sudah menggunakan obat hipertensi teruskan penggunaannya
secara rutin
6) Diet garam serta pengendalian berat badan
7) Periksa tekanan darah secara teratur
g. Komplikasi
Menurut Padila, 2013 Tekanan darah tinggi apabila tidak diobati dan
ditanggulangi, maka dalam jangka panjang akan menyebabkan kerusakan
arteri didalam tubuh organ yang mendapat suplai darah dari arteri tersebut.
Komplikasi hipertensi dapat terjadi pada organ-organ sebagai berikut:
1) Jantung
Tekanan darah tinggi dapat menyebabkan terjadinya gagal jantung dan
penyakit jantung koroner. Pada penderita hipertensi, beban kerja
jantung akan meningkat, otot jantung akan mengendor dan berkurang
elastisitasnya, yang disebut dekompensasi. Akibatnya, jantung tidak
mampu lagi memompa sehingga banyak cairan tertahan diparu
maupun jaringan tubuh lain yang dapat menyebabkan sesak nafas atau
oedema. Kondisi ini disebut gagal jantung.
2) Otak
Komplikasi hipertensi pada otak, menimbulkan resiko stroke, apabila
tidak diobati risiko terkena stroke 7 kali lebih besar.
3) Ginjal
Tekanan darah tinggi juga menyebabkan kerusakan ginjal, tekanan
darah tinggi dapat menyebabkan kerusakan system penyaringan di
dalam ginjal akibatnya lambat laun ginjal tidak mampu membuang zat-
zat yang tidak dibutuhkan tubuh yang masuk melalui aliran darah dan
terjadi penumpukan di dalam tubuh.
4) Mata
Pada mata hipertensi dapat mengakibatkan terjadinya retinopati
hipertensi dan dapat menimbulkan kebutaan (Yahya, 2005).
h. Penatalaksanaan
1) Penatalaksanaan Nonfarmakologi
Penatalaksanaan nonfarmakologis dengan modifikasi gaya hidup
sangat penting dalam mencegah tekanan darah tinggi dan merupakan
bagian yang tidak dapat dipisahkan dalam mengobati tekanan darah
tinggi (Ridwanamiruddin, 2007). Penatalaksanaan hipertensi dengan
nonfarmakologis terdiri dari berbagai macam cara modifikasi gaya
hidup untuk menurunkan tekanan darah yaitu:
a) Mempertahankan berat badan ideal
Mempertahankan berat badan ideal sesuai body mass index (BMI)
dengan rentang 18,5-24,9 kg/m2 (Kaplan, 2006). BMI dapat
diketahui dengan membagi berat badan anda yang telah di
kuadratkan dalam satuan diet rendah kolesterol namun kaya
dengan serat dan protein (pfizerpeduli.com), dan jika berhasil
menurunkan berat badan 2,5-5 kg maka tekanan darah diastolik
dapat diturunkan sebanyak 5 mmHg (Radmarssy, 2007).
b) Kurangi asupan natrium (sodium)
Mengurangi asupan natrium dapat dilakukan dengan cara diet
rendah garam yaitu tidak lebih dari 100 mmol/hari (kira-kira 6 gr
NaCL atau 2,4 gr garam/hari)b(Kaplan, 2006). Jumlah yang lain
dengan mengurangi asupan garam sampai kurang dari 2300 mg (1
sendok teh) setiap hari. Pengaturan konsumsi garam menjadi ½
sendok teh/hari, dapat menurunkan tekanan sistolik sebanyak 5
mmHg dan tekanan diastolik sekitar 2,5 mmHg (Radmarssy,
2007).
c) Batasi konsumsi alkohol
Radmarssy (2007) mengatakan bahwa mengkonsumsi alkohol
harus dibatasi karena konsumsi alkohol berlebihan dapat
meningkatkan tekanan darah. Para peminum berat mempunyai
resikomengalami hipertensi empat kali lebih besar dari pada
mereka yang tidak minum minuman beralkohol.
d) Makan K dan Ca yang cukup dari diet
Pertahankan asupan diet pottasium (>90 mmol (3500 mg)/hari)
dengan cara konsumsi tinggi buah dan sayur dan diet rendah lemak
dengan cara mengurangi asupan lemak jenuh dan lemak total
(Kaplan, 2006). Kalium dapat menurunkan tekanan darah dengan
meningkatkan jumlah natrium yang terbuang bersama air kencing.
Dengan setidaknya mengonsumsi buah-buahan sebanyak 3-5 kali
dalam sehari, seseorang bisa mencapai asupan potassium yang
cukup (Radmarssy, 2007).
e) Menghindari merokok
Merokok memang tidak berhubungan secara langsung dengan
timbulnya hipertensi, tetapi merokok dapat meningkatkan risiko
komplikasi pada pasien hipertensi seperti penyakit jantung dan
stroke, maka perlu dihindari mengkonsumsi tembakau (rokok)
karena dapat memperberat hipertensi (Dalimartha, 2008).
Nikotin dalam tembakau membuat jantung bekerja lebih
keras karena menyempitkan pembuluh darah dan meningkatkan
frekuensi denyut jantung serta tekanan darah (Sheps, 2005). Maka
pada penderita hipertensi dianjurkan untuk menghentikan
kebiasaan merokok (pfizerpeduli.com).
f) Penurunan stress
Stress memang tidak menyebabkan hipertensi yang menetap
namun jika episode stress sering terjadi dapat menyebabkan
kenaikan sementara yang sangat tinggi (Sheps, 2005). Menghindari
stress dengan menciptakan suasana yang menyenangkan bagi
penderita hipertensi dan memperkenalkan berbagai macam metode
relaksasi seperti yoga atau meditasi yang dapat mengontrol sistem
saraf yang akhirnya dapat menurunkan tekanan darah

g) Terapi masase (pijat)
Menurut Dhalimartha (2008), pada prinsipnya pijat yang dilakukan
pada penderita hipertensi adalah untuk memperlancar aliran energi
dalam tubuh sehingga gangguan hipertensi dan komplikasinya
dapat diminimalisir, ketika semua jalur energi terbuka dan aliran
energi tidak lagi terhalang oleh ketegangan otot dan hambatan lain
maka risiko hipertensi dapat ditekan.
2) Pengobatan Farmakologi
a) Diuretik (Hidroklorotiazid)
Mengeluarkan cairan tubuh sehingga volume cairan ditubuh
berkurang yang mengakibatkan daya pompa jantung menjadi lebih
ringan.
(1) Penghambatan Simpatetik (Metildopa, Klonidin, dan Reserpin)
Menghambat aktivitas saraf simpatis.
(2) Betabloker (Metoprolol, Propanolol dan Atenolol)
(a) Menurunkan daya pompa jantung.
(b) Tidak dianjurkan pada penderita yang telah diketahui
mengidap gangguan pernapasan seperti asam bronkial.
(c) Pada penderita diabetes melitus: dapat menutupi gejala
hipoglikemia.
(3) Vasodilator (Prasosin, Hidralasin)
Bekerja langsung pada pembuluh darah dengan relaksasi otot
pembuluh darah.
(4) ACE inhibitor (Captopril)
(a) Menghambat pembentukan zat Angiotensin II.
(b) Efek samping: batuk kering, pusing, sakit kepala dan lemas.
(5) Penghambat Reseptor Angiotensin II (Valsartan)
Menghalangi penempelan zat Angiotensin II pada reseptor
sehingga memperingan daya pompa jantung.
(6) Antagonis kalsium (Diltiasem dan Verapamil)
Menghambat kontraksi jantung (kontraktilitas).
