    The students can keep the documents above the black bar (no signature needed) ALL of the forms listed for each teacher should be given to the participating student at the beginning of the semester.
    Items below the black bar must be returned to our office after filled out (&/or signed) by student They will return them in 2 full sets (midterm and end of semester )
