AFFIDAVIT OF DISCREPANCY

I, JALIKA AMBAYANAN DANGANAN, of legal age, married, Filipino

and a resident of Tocawal, Alabel, Sarangani Province, after having been duly
sworn to according to law, depose and state that:

I am married to Mantil Lumayo Sumangka and blessed with ten children;

The youngest of our ten children is our daughter WARDA DANGANAN

SUMANGKA who was born on August 16, 1990 at Tokawal Alabel, Sarangani
Province;

Due to sheer negligence, I have registered the birth of my daughter Warda

only on October 12, 2006 in the Local Civil Registry of Alabel, Sarangani
Province;

In the tardily secured birth certificate of live birth of Warda, her gender was
erroneously indicated as “Male” instead of “Female” as her true and correct
gender;

My daughter Warda was born female and has not undergone sex change or
sex transplant;

Warda who is presently working abroad intends to apply for correction of

this error when she gets back to the Philippines considering that she can hardly
visit the office of the consul general for this purpose given her working condition
as domestic helper; and

I hereby executed this affidavit to attest to the truth of the foregoing and to
support the application of Warda for the renewal of her Passport and Visa.
