PETITION

Petitioner SAMBITORY C. LAGO JR., of legal age, married,

Filipino and a resident of General Santos City, unto this Honorable Court,
respectfully alleges that:

1. He is a citizen of the Philippines, thirty-one years of age having

been born on October 23, 1988 and a resident of Lot 13 Block 6
Zone XI-B Brgy. Fatima, General Santos City with mobile
phone number 09177776317;

2. He was admitted to the Bar on April 29, 2018 with Roll

Number 9260217 as evidenced by the Certificate of Admission
to the Philippine Bar issued by the Bar Confidant of the
Supreme Court and that there is no pending administrative case
against him as evidenced by the Certification to that effect also
issued by the Bar Confidant (Certifications are attached as
Annexes “A” and “B”);

3. He has paid his Professional Tax Receipt with number 6612185

on January 11, 2018 and IBP dues for 2019 with receipt number
1039579 on October 10, 2018 (Copies of receipts are attached
as Annexes “C” and “D”);

4. He was issued a certification by the IBP-South Cotabato and

General Santos Chapter Treasurer that he has paid the local IBP
dues (Copy of the certification is attached as Annex “E”);

5. He was issued a certification of good moral character by two

 executive officers of IBP-South Cotabato and General Santos
(Copy attached as Annex “F”);

6. He has completed the second compliance period of the

Mandatory Continuing Legal Education (MCLE) with MCLE
Compliance No. V-0008203 (Certificate of Compliance is
attached as Annex “G”);

7. He secured a clearance from the National Bureau of

Investigation (Copy attached as Annex “H”);

8. On May 29, 2018, he was commissioned by this Honorable

Court as a notary public for a term ending December 31, 2019
(Notarial Commission is attached as Annex “I”);

9. The petitioner is applying for the renewal of his commission as

Notary Public for and in the City of General Santos for a term
ending on December 31, 2020;

10.He possesses all the qualifications and none of the

disqualifications for said office;

11.He has never been convicted of any crime involving moral

turpitude;

12. His three passport size photographs, three specimen of his

signature and true impression of his dry seal made pursuant to
law are attached as Annex “J”); and

13.He already submitted his notarial files to the Office of the Clerk
of Court, General Santos City (Copy of transmittal is attached
as Annex “K”).

WHEREFORE, it is most respectfully prayed unto this Honorable

Court that this petition be granted.
