AFFIDAVIT
(Change Body Type and Color)

I, BONIFACIO A. BERNALDEZ, of legal age, married, Filipino

and a resident of Tocawal, Alabel, Sarangani Province, Philippines, after
having been duly sworn to according to law, depose and state:

That I am an automotive mechanic by occupation and presently

operating an auto repair shop specializing in body repair and painting jobs
of motor vehicles and particularly situated in Tocawal Alabel, Sarangani
Province;

That I was hired by Atty. Zavier A. Palao to change the body type and
color of motor vehicle registered in his name and more particularly
described below, to wit:

MAKE : ISUZU ELF

BODY TYPE : STEEL CAGE
ENGINE NO. : 4BC2185290
CHASSIS NO. : NKR55E 7107577
PLATE NO. : UCB 255
COLOR : WHITE

That I changed the body type of the aforesaid motor vehicle to

STEEL GRILLS and its color to BLACK; and

That I execute this affidavit to attest to the truth of the foregoing and
for whatever legal purpose it may serve.
