AFFIDAVIT OF LOSS

I, RAZEB D. BENITO, of legal age, married, Filipino and a resident

of Tocawal, Alabel, Sarangani Province after having been duly sworn to
before law, depose and state:

That I am a holder of a professional driver's license issued on January

2013 in General Santos City;

That on February 16, 2014, my wallet containing among others my

driver's license was taken by a pickpocket while I was in the public market
of General Santos City;

That I already considered my driver's license lost it being beyond

recovery given the circumstance;

That said driver's license was neither confiscated by any lawful

authority nor used in any unlawful act; and

That I hereby execute this affidavit to attest to the truth of the

foregoing and to support my application for replacement of my driver's
license.
