COMPLAINT-AFFIDAVIT
(Re: Violation of B.P. BLG. 22)

COMPLAINANT, assisted by counsel and unto this Honorable Office,

respectfully alleges that:

1. Complainant is of legal age and a resident of Diamad St., Purok

Camachili, Tambler, General Santos City where she may be served
with notices;

2. Respondent is also of legal age and presently the manager of

_____________ where she may also be served with notices;

3. On March 27, 2013, the respondent obtained from the complainant

five (5) items of jewelries worth Five hundred five thousand pesos (P
505,000.00) Philippine Currency payable in installment basis;

4. To ensure payment of her obligation, respondent issued to me ten (10)

post dated checks. Copies of said checks are attached as Annex “A”;

5. Upon presentment for clearing/collection, the checks were dishonored

by the drawee bank for the reason “Account Closed”;

6. Notice of Dishonor and Demand for payment was made upon

respondent, but _____________________________

WHEREFORE, it is most respectfully prayed of this Honorable Office that

a case of ESTAFA under Article 315(2)(d) of the Revised Penal Code be filed
against the respondent.
