REQUIREMENTS IN ISSUANCE OF NEW OWNER’S COPY OF TITLE

1. Certified true copy of the Title (must have the Affidavit of Loss annotated at the
back)
2. Plan (if any)
3. Tax Declaration (new)
4. Tax Declaration (old)
5. Updated Tax Receipt 2010
6. Certificate of no tax delinquency
7. Deed of Sale or any instrument vesting ownership and title
8. Certification from the Register of Deeds that their copy of the title is intact in their
office

Other data needed:

9. Name of owner and spouse, postal address, data on the valid ID


10. Name of adjoining owners, postal address
11. Such other documents proving ownership and possession
