This is to certify that the electrical facilities, safety protective devices, sizes of wires and
equipment installed of the existing 1 Storey Residential Commercial bldg. located at B3 Lot 7
Albany St. Vermont Village Mayamot Antipolo City has undergone regular preventive
maintenance and in condition, also in safe operating condition in such complies with the latest
provision of the latest edition of the Philippine Electrical Code, National Building Code and the
laws and Ordinances of the City of Antipolo

Generally under normal condition the electrical loads and installation of the
establishment is considered safe and stable for operation.

This certification was issued upon the request of the owner for whatever legal purpose
it may serve.
