You are a full‐time employee of the “The Clothing Outlet” in their Queen St store. The store is
located at 325 Queen St, Brisbane, 4000. The store’s telephone number is 07 1234 1234.
Petrov Slovak is the On‐Duty Supervisor. This morning at 10.15am, while you were sorting stock
in the storeroom, you tripped on a loose carpet tile. You fall and land heavily on your hands. No
one saw this happen and there no damage to stock. You don’t think you have broken anything,
but your wrist is swollen and very sore.

TASK 4 – Case studies
Purpose You will demonstrate your skills and knowledge by completing two
(2) Case Studies.
Case Study 1 You work as a cleaner for Absolute Cleaning Services. It’s 9pm and
Scenario you have arrived for work at Export Lawyers Pty Ltd, 29 Queen
Street, Brisbane, 4000. You find the cleaning equipment room in a
mess. The lids are off some of the chemical containers. The
vacuum cleaner is in a pool of liquid, and the plug is wet. You are
not sure what the liquid is, but it has a very strong smell.
Two other cleaners, Tom and Harry, are working on the same shift
and floor as you. The shift leader, Francis, is on a different floor.
Case Study 2 Jones Construction have expanded their business. They have
Scenario employed three new apprentices and a receptionist, Jenny.
During the first week, new staff are required to attend an induction
program. The program explains to them their role, how the company
operates, the different people and their roles, and organisational
policies and procedures.

Case Study 2
Read the Case Study 2 Scenario above and the situation below. Complete questions 1 to 6.
During the induction, Jenny, the new receptionist, asks the following question, “If I identify a risk
or hazard in my work area or if there is an emergency, who should I contact?”
1. Identify two (2) people that Jenny should contact about this hazard.
It’s been two weeks since Jenny and the apprentices completed their induction. Jenny’s
supervisor sits in a nearby office. Qualified tradesmen on the job sites supervise the
apprentices.
Jenny has been enjoying her role at Jones Construction. She is responsible for accepting
deliveries from couriers. She has noticed that the couriers leave boxes just inside the doorway
and on a number of occasions staff have nearly fallen over the boxes.
2. What is the potential hazard that Jenny has identified? Where should she record this hazard

TASK 5 – Role Play & Observation
Purpose An assessor will observe and assess your performance. You will perform
a role‐play in a simulated workplace environment.
Role‐Play & You will perform this task with your trainer. The role‐play scenario is

The role‐play will take 5 ‐ 10 minutes. You will need to perform the role‐

knowledge according to the checklist below.
Please practice the role‐play and make sure you are covering the
required areas in the role‐play. Use the observation checklist as your
guide.
