PROGRAM PERFORMANCE PROFILE

A. Administration

 Organizational structure of the library

The Associate Director of Library Services is directly under the


administration and supervision of the Director of Library Services from Bayombong
campus and supervised by the Vice President for Academic Affairs and also
directly under the management of Campus Administrator. She directly supervised
the library staff.

The Director of Library Services and the Vice President for Academic
Affairs is in coordination with the Campus Administrator.

The Campus Administrator is directly under the office of the president.

The Library Advisory Council/Committee is chaired by the Vice-President


for Academic Affairs with the Campus Administrator, Deans of different colleges
and the designated Associate Director for Library Services as members.

The library organizational structure is revised/updated as the designated


officials will end their terms every after two (2) years or as earlier revoked.

The functions of the Library Advisory Committee are the following:

1. Help in the selection of library materials to be acquired in their respective


field of specialization.

2. Serve as channels in informing and updating the library on research,


extension, and instructional needs.

3. Help in the formulation of policies relative to the development of resources


for instruction and research.

 Description of the Head Librarian in terms of qualification,


supervisory functions and participation in academic and
administrative activities of the Institution.

The Head Librarian is a holder of Bachelor of Arts major in Library Science,


at Saint Mary’s College, Bayombong, Nueva Vizcaya, a Master in Educational
Administration major in Library Science in the same school now Saint Mary’s
University, and a Doctor of Philosophy in Organization Development and Planning
here at Nueva Vizcaya State University, Bambang, Nueva Nueva Vizcaya. She is
a licensed librarian with PRC license number 0002682 average rating of 83.60%;
she took the Board for Librarianship Examination last Nov. 21 & 22, 2000 at
MLQU, Manila; she was also PBET Passers now LET examination with an
average rating of 76.73% taken last May 30, 1993 at NVGCHS, Bayombong,
Nueva Vizcaya. Twenty (20) years experience in librarianship with relevant
training in library and information management, at present she is a member of the
Accrediting Agencies of Chartered Colleges and Universities in the Philippines Inc.
(AACCUP) since June 2008, and member of the Internal Assessment Body (IAB)
of the university for Bambang and Bayombong campus.

Library

The head librarian is in-charge of the overall supervision of the university


library. Technical assistance and supervision is also extended to the mini-library
of the different colleges. She is directly in-charge of the Technical Processing of
the different collections in the library. Under her supervision is the in-charge of the
Graduate School Section, Periodical, Laboratory, Audio-Visual, and the General
Circulation Section.

Inherent to her function as head librarian, she coordinates with the


Department Head for Languages in the conduct of library orientation for the
freshmen and transferees students every beginning of school year, and also take
charge in the dissemination and implementation of library policies.

She is also responsible in attending meetings conducted by the higher


authorities and attends open forum as requested by the student body. Previous
years had a teaching load at the Graduate school program for the Ph.D., MAED,
MAT, and MPA programs.

The head librarian is classified as academic non-teaching staff (College


Librarian I) plantilla item. A member of the (CODD) Council of Deans and
Directors, (ADCO) Administrative Council, and the Library Board
Council/Committee which work towards the continued development and
improvement of the university library.

 Policies and procedures on the administration of the library

The university library is administered and supervised by a College librarian I


plantilla position and is designated as Associate Director, Library Services with the
assistance of two (2) professional licensed librarians (College librarian III and
College librarian I) plantilla position. Six (6) student assistants were also hired to
help the staffs in the charging of library materials and other services and in the
maintenance and upkeep of the university library.

The designated Associate Director for library services is a fulltime librarian


who supervises the whole operation of the library while one faculty member of
the college was assigned to take charge of the mini-library (Technology and
Engineering Mini-Library). The library determines the resources and equipment
needed based on the need assessment, recommendation and suggestion by the
library staff, faculty and students. The Library Advisory Council/Committee was
organized to serve as an advisory body and help formulate policies and
regulations for the university library. These policies and regulations are embodied
in the Library Manual to serve as a guide in the management and operation of the
library.

The NVSU library, maintains a live and growing collection; a modern


physical plant; a well-rounded staff; leadership in promotion of library use; and
adequate financial support. Systematically arranged and organized for easy
retrieval. Included under administration and supervision of the library would be
programming, scheduling, staffing, record keeping, monitoring of library services,
maintaining good public relations, and promoting the use of the library. Aside from
the functions on (POSDCORB) Planning, Organizing, Staffing, Directing,
Controlling, Reporting and Budgeting added to this is evaluating. (Attached the
library general policies)

LIBRARY GENERAL POLICIES

I. Issuance of Library Card


Application for library card must be made upon enrollment (follow
Enrollment Procedure)

1.1 Requirements for new and old students


1.1.1 Student copy of duly accomplished Registration
Form/Assessment Form
1.1.2 Latest 1”x1” ID picture (1 pc.)

1.2 Requirements for lost library card


1.1.1 Pay a library card replacement fee of P20.00 to Cashier’s
Office.
Present the official receipt of payment to the Librarian for the
issuance of new library card.

2. Required Conduct within the Library Premises


2.1 While within the premises of the library, silence must be observed at all
times. Violators will be asked to leave the
library premises.
2.2 Smoking, littering, sleeping, eating and drinking are strictly prohibited.
2.3 Lending library card to other students for their use; tampering of library
cards, forging signature and other records are causes for the
confiscation of university ID and library card. Vandalism/destruction of
library properties/materials is strictly prohibited.
2.4 Upon entering the library, leave your things on the Depository Area.
Only sheet of paper & valuable materials allowed to be taken inside;
otherwise, the library is not responsible for the lost of such articles.

3. Return and Recall of Books


Any book on loan may be recalled at anytime. Recalled materials should
be returned at once.

4. Mutilation and Stealing of Library Materials


Any student who shall deface, mutilate, appropriate for himself, or steal any
library material or property shall be required to replace the same or pay the
replacement and processing costs thereof and shall be subject to a fine of not less
than P50.00 nor more than P300.00 to be determined by the Librarian without
prejudice to the imposition, in appropriate cases, of the additional penalty of
suspension or expulsion upon the recommendation of the Librarian to the
President, through the Dean concerned.

5. Neglect to Settle Library Accounts


5.1 A student who refuses or fails to settle his/her outstanding library
accounts shall face any or all of the following penalties:
5.1.1 Not allowed to borrow books and other materials;
5.1.2 Not permitted to use the University Library and its facilities;
5.1.3 Recommended for suspension/expulsion from the University.
5.2 The following rates of fines shall be charged to library users;
5.2.1 The fine for the late return of book is P5.00 for the first hour
and P1.00 for each succeeding hour.

6. Signing of Clearance
Students must secure clearance every end of semester and it is mandatory
to pass through the Office of Library Services. Likewise in applying for Diploma,
Transcript of records and Honorable Dismissal.

7. Borrowing Books and other Library Materials


7.1 A student who borrowed a book/periodical must assume full
responsibility for the proper care of the borrowed materials. A book/periodical
must not be marked, defaced or mutilated.
7.2 The presentation of validated Library Card to the librarian is required
when borrowing books and other library materials.
7.3 Loaning out books for home use starts at 2:00 PM and should be
returned not later than 8:30 AM of the following day.
7.4 Library users shall be subjected to fines for overdue books.
7.5 Books shall not be allowed for home use two weeks before final
examination period.
7.6 Periodicals & Reference Books are strictly for “ROOM USE ONLY”.
7.7 Catalogue cards are not to be withdrawn from the library catalogue
cabinet.
7.8 In the Graduate Section, graduate students are allowed to borrow two
(2) books for home use in any given week.

8. Lost Books and other Library Materials


8.1 Lost books and other library materials shall be immediately reported to
the library.
8.2 Any person who loses or fails to return a book within seven days shall
either replace it within thirty days or pay its current value together with the accrued
fines.
8.3 Payment of lost materials shall not exceed the current value of the
material plus processing fee of thirty (P30.00) pesos. If the book is out of print, the
cost shall be charged in lieu of the current value.

9. Non-compliance with Library Rules and Regulations


Any non-compliance with library rules and regulations shall be subjected to
sanctions/penalties depending on the gravity of the infraction.

10. For Non-NVSU users


Non-NVSU users are required to secure the following before they are
allowed to use the NVSU library and its facilities:
10.1 A referral letter issued by the head/librarian of the university/agency
where they come from.
10.2 Payment of library fee of P20.00 to be paid at the Cashier’s Office.
10.3 Official receipt of payment to be presented to the librarian and a
visitors ID will be issued to the non-NVSU user.

 Brief Outline of the Library Staff Development Program

LIBRARY STAFF DEVELOPMENT PROGRAM


 
As strategy towards an efficient and effective library services, the university
library should have a sufficient number of staff to organize and maintain the
collection as well as to manage information and reference services for the
clientele.
At present the university library, staffed with three (3) professional, licensed
Librarians. Two finished Doctorate Degree and one with 18 units Masteral Degree
course.
 Assisted by six (6) student librarians with a maximum of four hours duty
per day. During the plan period (2009-2014), if budget permits with the existence
of automated library system the need to hire one licensed librarian to manage and
supervise the said integrated library system.
As mandated in the AACCUP instrument for accreditation, the Head
librarian preferably with an academic rank/faculty status. As a result of previous
accreditation, the head librarian requested for a faculty status, it was approved by

the university administrators pending for the availability of faculty plantilla item.
Also, opportunities for seminars, trainings are provided for all the library staff either
local, national or even international conferences.

Before the year 2014, Mrs. Edna T. Valmores, College Librarian I is

 The Institution’s policies and procedures, collection development,


organization and preservation of library materials and resources.

COLLECTION DEVELOPMENT
The Library collection is intended to be a highly selective basic level
collection but sufficient enough to support curricular offerings as determined
by the university’s mission, goals, and objectives, and by size and scope.
Materials include such resources as major dictionaries, encyclopedias,
historical surveys, monographs, books, fiction, pamphlets, journals, archival

material, audio video materials, bibliographies, and periodicals in various


formats including print and electronic. Beyond the scope and content of the
collection, other factors such as cost, usability, and currency determine
acquisition priorities.
The university officials, professional librarian, faculty, staff, students,
and the community have the responsibility for the Collection Development
of the university library. (Library CMPS and Book Collection Development

ORGANIZATION AND PRESENTATION


The preparation and organization of library books are processes
performed by the Technical Services of the library. In the Bambang
campus, it is done by the Associate Director, though in the Laboratory High
School collection, there is a separate accession records, the same
classification procedure is done.

Cataloging and classification are tasks done solely by the


professional staff. The NVSU library Bambang campus officially uses the
following tools: Anglo-American Cataloging Rules 2 (AACR2); Sears List of
Subject Headings (SLSH), 18 th ed.; Dewey Decimal Classification System,
21st ed. (DDC 21st ed.); and the C.A. Cutter’s Three Figure Table. These
are done in a 3’5” P-slips which serve as shelf-lists and are a basis in
preparation of the three cards Author, Title, and Subject cards which are
arranged alphabetically in the Card Catalog Cabinet and used for encoding
purposes.

PRESERVATION OF LIBRARY MATERIALS AND RESOURCES


The library strives to maintain the physical integrity of the materials
through preservation procedures such as proper handling and storage, and
security program. Back issues of periodicals, journals, magazines are
bound and indexed. Simple mending of books is done by the librarian in-
charge.

D. Services and Utilization

 Description of procedures in the utilization of the library resources


and availment of personnel services.

The university library provides various services to the academic


community through the cooperative effort of many people.

The University Library adopts Open stock system which provides


faculty, students and other users’ greater access to the collection and
services. Open stock system is used in the different sections of the library
except the General Circulation & Laboratory Section where in semi-open
stock system is adopted. The card catalog is properly maintained and
updated with title, author, and subject cards for each entry. Newly acquired
books and other library materials are display in the “New Arrivals” shelve for
one week, for browsing before it will be transferred to the different sections
of the library.

Audio-visual room is readily accessible to individual adjacent to the


library. Proper scheduling and coordination is properly done to the librarian
in-charge.

A separate non-reading area is also designed beside the audio-


visual room for every library users for relaxation and study.

The existence of separate Museum located at the left wing of the


university library was properly designed and organized for the whole
populace convenience in locating artifacts or any related materials,
information, managed and supervised by faculty member designated by the
university.

All incoming students are given library orientation where library


services, policies and regulations on how to use the card catalog, e
resources/internet services are properly discussed and demonstrated;
actual tour regarding the different sections of the library is properly
administered.

An approved Library Manual, Policies, Rules & Regulations,


Collection Management Policy Statement in Selecting, Acquiring and De-
selecting library materials, and Book Collection Development Program that
guides its internal administration and operation is available. A library flyer is
given to each student during the library orientation and Library Bulletin
boards are regularly structured and updated.

The library is open from 7:30-5:00 in the afternoon “NO NOON


BREAK” daily from Monday to Friday and during Saturday 8:00-5:00
catering the whole populace of the university with a total of 56.5 library
hours per week. A logbook is used to monitor the daily attendance of
library clients per college and statistics on the utilization of library resources
is maintained.

Library Newsletter is regularly published to promote and


disseminates regular announcements of its new acquisitions, resources,
facilities, and services. Library staff are ready available during library
hours to assist and provide library services to users/clientele.

 Evidence/s of the extent of utilization of the library by clientele and


end-users.

Logbook for library users/clientele categorized with the different


colleges were compiled and presented in the graphical form for easy
identification who among the different colleges are frequent users of the
library.

Statistical records for the different subject areas book borrowed were
also compiled and also available in graphical format.

Records on the utilization of the Audio-Visual room, Periodical


section are also recorded in a logbook/record book properly compiled and
presented in a graphical presentation.

Proper scheduling on the use of Audio-Visual room is done to avoid


conflicts among users/clientele as evidenced by the logbook for AVR.

Logbook on the utilization of internet services is also maintain and


kept properly for future reference.

Use of books and other library resources were properly charged,


signed with the book cards as evidenced a certain book is
borrowed/utilized.

Request letters are also compiled in borrowing chairs, tables


utilized/use for academic and non-academic programs in the university.

Issuance of Referral letter to students/clients who want to avail


libraries outside Nueva Vizcaya State University library Bambang campus
was also maintained as evidenced by the logbook requesting referral letter.

E. Physical Set-up and facilities

 Description of the set-up (e.g. location and site, space requirement


for the different sections, etc.)

Location and site

The university library is centrally located at the 3 rd floor of the College


of Teacher Education Building. The building is near the Administration and
College of Arts and Sciences building overlooking the stage of the

university quadrangle. Below are the stalls of Xerox copier and pathway
going to Testing Center, Bakery and Medical/Dental Clinic.

Space requirement for the different sections

Space for the Audio-Visual Room, Museum, Non-reading Area,


Stock Room, Internet area / Electronic Resources, Technical Room, Staff
Room, Entrance and Exit Area, and a ramp for physically disabled clientele
are provided.

Its floor areas are 68m x 8m = 544m 2, for the reading area and the
Audio-Visual room which is adjacent to the right wing of the library is 12m x
10m=126m2 and can accommodate 272 clients at a time.

The library is adequately lighted and ventilated. Air condition units,


Ceiling fans are properly installed to the different sections of the library; it
has also enough windows for natural ventilation and outfitted with curtains
for comfort and convenience of users.

Provision for a separate building in the near future is a need to house


the growing number of collection and the e-library resources.

 Description of other provisions (e.g. security/control, IT software,


multimedia equipment, etc)

Security/Control

Safety measures are also considered to safeguard the university


library. A student librarian or library staff is designated at the depository
area to look after the belongings of the clients with corresponding number
when they leave or deposit their bags at the counter for security purposes.

Counter checking of the book borrowed is done by checking the date


due slip of the book and proper signature of the charger in-charge is also
counter check to avoid losses of books and other library materials.

Provision for local alarm system, detector device, or CCTV camera


to safeguard the library resources is needed.

IT Software

IT software intended for library automation was developed and


designed by faculty member of this university, Chairman of Information
Technology Department, in-charge of the e-lab and he also developed the
MAK system of the university both Bambang and Bayombong campuses
uses the said developed software as attendance record keeping to all
personnel of the university in time- in and time- out purposes.
Booktrucker was developed intended for the various collection of the
library for borrowing and returning of books and the library web page was
also developed aside from the university web page.

Multimedia Equipment, etc.

Micro-reader, Opaque projector, 16mm film projector, Laserdisc player,

Scanner, Binding machine, and Xerox copier, is needed but it needs


additional budget for library services.
At present the university library has netbook, set of computers and
printers, overhead projector, LCD projector, number of CDs, VHS tapes,
VHS rewinder, slides, films, Digi Cam, etc. available for use.

 Description of technology utilization in library operations such as:

 Integrated library system

The integrated library system of the university was not functional in


which books and other library resources are still on the process of encoding
and bar coding of books for easy retrieval of library collection. Books with
corresponding barcode, just type the subject or title of books it will appear
the bibliographic description of a certain book and if you want to borrow just
swipe it in the barcode scanner and automatically you can now charge out
the books with a receipt.

 Access to electronic collections (e.g. books, e-journals, internet


access)

E-books and subscription to e-journals are not yet started, hopefully


by next school year were able to start at least one title of books and
journals for every programs of the university. At present only one title of e-
journal had been subscribed online (Journal of Electronic Materials)
Internet access in the library is also available, nine (9) units of computers
readily accessible for internet purposes with printer for faculty, staff and
students and even for visitors use and must pay the corresponding fee at
the cashier office. It is also available during Saturdays for Graduate School
students and the university library is a Wi-Fi zone if you have laptop you
can access internet with the permission from the librarian in-charge and at
the ISSC Office.

 Access to online database and e-resources

Through internet we can access different e-resources of different


agencies with their corresponding websites.

 Institutional repositories

For institutional repository we have the MIS Office to collect, deposit,


manage and restore needed information in the university.

Note: A total of three hundred thousand (P300,000.00) was allotted for book
purchased regularly every year, it was divided equally among the different
colleges (Arts and Sciences, Education, Engineering and Technology), the

Budget Preparation

The university allocates a regular and realistic budget for the library. A
university budget meeting was conducted regularly every start of the year to
discuss financial matter for every department, colleges, and units/section.

The Associate Director for library services, in coordination with other


officials of the university, prepares and manages the annual library budget.

The (APP) Annual Procurement Plan is made every start of the calendar
year prepared by the Associate Director, Library services in consultation with the
library staff, a separate budget for supplies and equipment was prepared. Budget
for books, subscription to journals/periodicals, trainings/seminars for library staff,
wages of staff, multimedia materials, repair, binding and maintenance was also
properly allocated.

Summary of Findings and Recommendations for Area VII


(Library) in the Self-Survey:

1. Strengths

1. The university library is managed by a licensed, educationally


qualified librarian with the expertise in library administration and
supervision.

2. The head librarian directs and supervises the total operation of the
library and is responsible for the administration of its resources and
services.

3. The annual budget allocation of the library is solely used for the
utilization of library growth and development.

4. The annual reports, accomplishment reports and other reports of the


library are promptly submitted to the higher offices concerned.

5. The Library Committee plays an active role in library policy


formulation and assist in book selection, book acquisition and
evaluation.

6. There is a well-defined collection development policy.

7. The library adopts system which provides faculty, students and


other user’s greater access to its collection and services.

8. The library promotes and disseminates its program through a


regular announcement of its new acquisitions, resources, facilities,
and services as evidence the Library Newsletter, which is the official
Newsletter of NVSU library.

1. Enhance the self developed integrated library system to facilitate


library services.

the school population.

journals to upgrade the present resources in the library.
6. If budget permits, replacement of the existing sub-standard
shelves to provide wider space to collection for easy re-
arrangement of the physical set-up of the library.
